肩关节正位X线摄影技术的改良与应用

【摘要】：目的探讨肩关节正位X线摄影技术的3种投照方法与临床应用。方法选取医院2015年9月至2016年4月拍摄肩关节正位的患者109例;肩关节正位3种常规投照位置分别为内旋位、外旋位和中间正位像,统计检查结果。结果所示患者中类风湿性关节炎16例,骨关节炎11例,冻肩4例,滑囊炎5例,Bankart损伤3例,Hill-Sachs缺损2例,骨质疏松症16例,未见明显异常的62例。其中,内旋位的正位像适用于证明习惯性肩关节脱臼出现的肱骨头后外侧部的骨缺损(Hill-Sachs病);外旋位能较好地显示肱骨头的形态,对于大结节骨折和冈上肌肌腱钙质沉着的诊断有临床意义;中间正位可以显示肱骨近段和上肢带骨的骨折和(或)脱位,还可以显示肌肉,肌腱或关节囊结构的钙质沉积,诸如骨质疏松和关节炎等病理改变得到很好的显示。结论按照上述所说体位进行肩关节正位投照,可获取高质量的影像图片,有利于医师完成明确地诊断和精准治疗。
[Abstract]:Objective to explore three methods and clinical application of shoulder orthographic radiography. Methods 109 patients with shoulder position from September 2015 to April 2016 in our hospital were selected. The results showed that 16 cases of rheumatoid arthritis, 11 cases of osteoarthritis, 4 cases of frozen shoulder. There were 3 cases of Hill-Sachs defect and 16 cases of osteoporosis in 5 cases of bursitis with Bankart injury and 62 cases with no obvious abnormality. The positive images of internal rotation can be used to prove the Hill-Sachs disease in the posterior lateral part of the humerus head, which is proved by habitual dislocation of shoulder joint. The external rotation can show the shape of humerus head well and has clinical significance for the diagnosis of great tubercle fracture and calcinosis of supraspinatus tendon. The middle position shows fractures and / or dislocations in the proximal humerus and the upper limb, as well as calcium deposits in muscle, tendon, or articular capsule structures. The pathological changes such as osteoporosis and arthritis are well displayed. Conclusion according to the position of the shoulder, we can obtain high quality image of shoulder joint. It is helpful for doctors to complete definite diagnosis and accurate treatment.
