运动性肩袖损伤表面肌电及等速肌力测试研究

【摘要】:目的:探讨分析运动性肩袖损伤后肩袖肌群表面肌电和等速肌力测试特征的变化。方法:选取肩袖损伤的运动员30例,其中男18例,女12例,平均年龄为21.2±2.2岁。同时选取同一院系年龄、性别构成比相近的健康运动员30例作为对照组,其中男16例,女14例,平均年龄为21.5±2.0岁。两组患者均同时进行表面肌电和等速肌力测试。结果:肩袖损伤组运动员肩关节在角速度为60°/s,90°/s外展时冈上肌、冈下肌积分肌电值(i EMG)值比对照组低,差异有统计学意义(P0.05),尤其是冈下肌差异更显著(P0.01)。肩袖损伤组运动员肩关节在角速度为120°/s外展时冈上肌、冈下肌i EMG值比对照组低,差异有统计学意义(P0.01)。肩袖损伤组运动员肩关节在角速度为60°/s外旋时所有肌肉i EMG值与对照组比较,差异无统计学意义(P0.05);肩袖损伤组运动员肩关节在角速度为90°/s、120°/s外旋时冈下肌i EMG值比对照组低,差异有统计学意义(P0.05)。与对照组比较,肩袖损伤组运动员肩关节在角速度为60°/s,90°/s,120°/s外旋时所有肌肉i EMG值均无明显改变(P0.05)。肩袖损伤组运动员肩关节在角速度为60°/s,90°/s,120°/s时的外旋/内旋,外展/内收峰力矩值比对照组低,差异有统计学意义(P0.05或P0.01)。结论:肩袖损伤运动员在外展时冈上肌、冈下肌肌肉募集较差,而三角肌、小圆肌无异常;同时,肩袖损伤后三角肌中束和冈上肌合力作用下降而致肩关节外展能力减弱。
[Abstract]:Objective: to investigate the changes of surface electromyography and isokinetic muscle strength of rotator cuff group after rotator cuff injury. Methods: 30 athletes with rotator cuff injury were selected, including 18 males and 12 females. The average age was 21.2 卤2.2 years. 30 healthy athletes with the same school age and similar sex ratio were selected as the control group, including 16 males and 14 females. Results: the shoulder joint of the rotator cuff injury group was 60 掳/ s at angular velocity. At 90 掳/ s abduction, the integral EMG (I EMG) of supraspinatus and infraspinatus was lower than that of the control group (P 0.05). In the rotator cuff injury group, I EMG of the supraspinatus muscle and the infraspinar muscle were lower than that of the control group at the angle velocity of 120 掳/ s. The difference was statistically significant (P 0.01). The I EMG values of all muscles in the rotator cuff injury group were compared with those in the control group when the angular velocity was 60 掳/ s. The difference was not statistically significant (P 0.05). In the rotator cuff injury group, I EMG was lower in the shoulder joint at the angle velocity of 90 掳/ s or 120 掳/ s than that in the control group, and the difference was statistically significant (P 0.05). The shoulder joint of the rotator cuff injury group was 90 掳/ s at the angular velocity of 60 掳/ s. There was no significant change in the I EMG value of all muscles during 120 掳/ s external rotation. The shoulder joint of the rotator cuff injury group was at an angular velocity of 60 掳/ s ~ 90 掳/ s. The external rotation / internal rotation and abduction / adductive peak torque of 120 掳/ s were lower than that of the control group (P 0.05 or P 0.01). Conclusion: the rotator cuff injury athletes' supraspinatus muscle. The muscle recruitment of the infraspinar muscle was poor, but the deltoid muscle and the lesser round muscle were not abnormal. At the same time, the joint action of deltoid muscle and supraspinatus muscle decreased after rotator cuff injury.
