乳腺影像报告和数据系统规范下的乳腺癌X线表现及病理基础

【摘要】：目的探讨乳腺钼靶X线摄像在乳腺癌诊断中的价值,加深乳腺影像报告和数据系统(BI-RADS)规范下的乳腺癌X线特征的认识,以期提高女性乳腺癌病灶的检出和评估。方法按照美国放射学会(ACA)创立并推荐的BI-RADS,回顾性总结2310例病灶的乳腺钼靶X线摄像特征,并对经病理证实的乳腺癌进行分级及年龄层分析。对94例有手术或穿刺活检证实的病例,对钼靶影像学征象、临床表现、年龄层、体重指数等多方面进行比较分析。结果(1)经过分类统计,2310例病灶中：0级为20例(0.87%),1级为321例(13.90%),2级为1105例(47.84%),3级为594例(25.71%),4a级为179例(7.75%),4b级为52例(2.25%),4c级为21例(0.91%),5级为1 8例(0.78%)。(2)94例经手术或穿刺活检证实,确诊良性54例(57.45%),恶性40例(42.552%)。(3)在40例恶性病灶中,X线显示单纯肿块16例(40.0%),肿块伴钙化14例(35.0%),不对称性密度增高5例(12.5%),仅有钙化者5例(12.5%),结构扭曲13例(32.5%)。结论乳腺钼靶X线摄像技术在BI-RADS的规范下,在乳腺良恶性疾病的鉴别中具有重要价值。
[Abstract]:Objective to explore the value of mammography in the diagnosis of breast cancer, and to deepen the understanding of mammography characteristics of breast cancer under BI-RADS specification. Methods according to the BI-RADS established and recommended by the American Society of Radiology, the mammography features of 2310 cases of breast cancer were retrospectively summarized. The grade and age of breast cancer confirmed by pathology were analyzed. In 94 cases confirmed by operation or puncture biopsy, mammography signs, clinical manifestations, age group were analyzed. Body mass index and other aspects were compared and analyzed. Results: 1) according to the classification of 2310 lesions, 20 cases were divided into 0 grade 0, 20 cases 0.87 cases, 321 cases with grade 1, 13.9090 and 1 grade 1, 1105 cases with grade 2, 47.84 and 3, 594 cases with grade 4a of 7.751a, 179 cases with grade 7.75, 4b, 52 cases with grade 2.2525, 4c, grade 21, grade 0.911J, 18 cases. 94 cases were confirmed by operation or puncture biopsy. In the 40 cases of malignant lesions, the X ray showed simple mass in 16 cases, simple mass in 16 cases, calcification in 14 cases (35. 0%), asymmetry density in 5 cases (12. 5%), calcification in 5 cases (12. 5%), structure distortion in 13 cases (32. 5%). Target X-ray camera technology under the BI-RADS specification, It has important value in differentiating benign and malignant breast diseases.
