气胸容积CT快速量化测定研究

【摘要】:目的利用胸部CT扫描轴位图像的四个固定层面胸膜间距离之和计算气胸的肺压缩程度,以探索基于CT轴位图像更简便、精确的气胸评价方法。方法回顾性分析2013年1月至2015年6月间本院73例气胸患者的胸部CT平扫资料,将层厚为2 mm的胸部CT数据导入东芝Aquilion ONE后处理软件Vitrea(Version6.5.8),测得的容积数据经计算得到的肺压缩值作为参考标准;将胸廓入口内缘至压缩肺尖边缘距离记为A,在胸部CT的固定三个解剖层面(气管分叉层面、嵴角层面、膈顶层面)分别标记压缩肺锁骨中线及腋中线的气体厚度(矢状面三线:沿锁骨中线矢状面测量三个固定解剖层面脏壁层胸膜间距离的三线分别记为B、C、D;冠状面三线:沿腋中线冠状面测量三个固定解剖层面脏壁层胸膜间距离的三线分别记为B~1、C~1、D~1),计算距离之和(A+B+C+D)及(A+B~1+C~1+D~1),将所得结果分别与CT容积测量法所测值进行直线回归分析,得出直线回归方程。结果根据肺压缩值与两组CT四线之和的结果回归分析,计算肺萎陷程度的公式为:矢状面三线Y=0.001×(A+B+C+D)+0.234(r=0.855,P0.001);冠状面三线Y~1=0.002×(A+B~1+C~1+D~1)+0.247(r~1=0.867,P10.001)。结论基于常规胸部CT扫描轴位图像测量的CT四线法是计算气胸肺萎陷程度方便而准确的方法,值得临床推广应用。
[Abstract]:Objective to calculate the lung compression degree of pneumothorax by using the sum of the distance between the four fixed layers of chest CT scanning axial images, so as to explore a more convenient method based on CT axial images. Methods the chest CT plain scan data of 73 patients with pneumothorax from January 2013 to June 2015 were analyzed retrospectively. Chest CT data with slice thickness of 2 mm were imported into Toshiba Aquilion ONE post-processing software Vitreae version6.5.8, and the calculated lung compression values were used as the reference standard. The distance from the inner edge of the thoracic entrance to the edge of the compressed pulmonary apex was recorded as A, and three anatomical levels (trachea bifurcation, cristae angle) of the chest CT were fixed. The gas thickness of midline and axillary line of compressed lung clavicle (sagittal three lines: three lines along the sagittal plane of the middle clavicular line measuring the distance between the three fixed anatomical layers of the visceral layer pleura were marked as BnCU D; coronal plane 3 lines respectively). Line: the three lines along the coronal plane of axillary midline for measuring the distance between the layers of the visceral wall on three fixed anatomical planes were recorded as BU 1C 1C 1D, calculated the sum of distances A B C D D) and A B 1 C 1 D D D 1, respectively. The results were analyzed by linear regression analysis with CT volume measurement. The linear regression equation was obtained. Results regression analysis was made based on the lung compression value and the sum of the two groups of CT four lines. The formula for calculating the degree of lung collapse is as follows: sagittal plane three-line YP0. 001 脳 A B C D) 0.234ru 0.855U P0.001; coronal plane 3- line Y1 0. 002 脳 A BX 1 C 1 D0 1) 0.247C 0. 867P10. 0011.Conclusion the four line CT method based on conventional chest CT scanning is a convenient and accurate method for calculating the degree of pneumothorax lung atrophy. It is worth popularizing and applying in clinic.
