28例肺原位腺癌低剂量扫描的CT表现及文献复习

【摘要】：目的探讨肺原位腺癌的临床和能谱CT低剂量扫描的影像学表现。方法回顾分析近3年来该院经手术病理证实的肺原位腺癌28例患者的资料,所有患者均行能谱CT低剂量扫描。结果 CT表现为混合磨玻璃结节影15例,磨玻璃结节影9例,实性结节影2例,模糊小斑片状密度增高阴影2例;病灶最大径均小于30 mm,其中最大径10~20 mm 22例。结节边缘模糊26例、清楚2例,分叶、毛刺20例,病灶内密度不均匀26例,均匀2例,可见局部支气管充气征15例。所有病例无一例发现远处或淋巴结转移。结论肺原位腺癌CT多表现为磨玻璃或混合磨玻璃结节影,小部分为实性结节影及小斑片状稍高密度影。能谱CT低剂量扫描能清晰地显示肺原位腺癌的影像学特征,对肺原位腺癌诊断具有重要的临床价值。
[Abstract]:Objective to investigate the clinical features of lung in situ adenocarcinoma and the imaging features of low dose scan with energy dispersive CT. Methods 28 cases of lung in situ adenocarcinoma proved by surgery and pathology in our hospital in recent 3 years were retrospectively analyzed. All the patients underwent low dose EDS CT scan. Results the CT findings were mixed glassy nodular shadow in 15 cases, milled glass nodule shadow in 9 cases, solid nodular shadow in 2 cases, and fuzzy patch density increased shadow in 2 cases. The maximum diameters of the lesions were less than 30 mm, in which the maximum diameter was 10 ~ 20 mm in 22 cases, the edge of the nodules was blurred in 26 cases, clear in 2 cases, lobulated and punctured in 20 cases, the density of the lesions was uneven in 26 cases, and even in 2 cases. No distant or lymph node metastasis was found in all cases. Conclusion the CT findings of lung in situ adenocarcinoma are mostly glass-grinding or mixed glass-grinding nodular shadow. The small part is solid nodule shadow and small spot shape slightly high density image. The low dose scanning of energy dispersive CT can clearly display the imaging features of lung in situ adenocarcinoma, which has important clinical value in the diagnosis of lung in situ adenocarcinoma.
