单臂冠脉造影三维重建与定量分析

【摘要】:冠脉造影是检查心血管疾病的最有效手段,,但其二维成像方式增加了读片难度并制约定量分析精度。三维重建为临床诊断与治疗提供了新的解决方案,可以提供直观真实的冠状动脉空间结构,方便医生定位病灶,精确测量血管参数。对造影序列进行重建还可以充分挖掘造影图信息,估计血管运动参数,并研究心脏运动情况。 本文首先分析了双视角三维重建原理,分析了影响重建精度的各种因素,提出了能够满足后续分析重建结果的优化方法。为了更好的表达重建的血管表面,本文拓展了常用的以圆来描述血管横截面的广义圆柱模型,利用参与重建的两幅造影图信息,以椭圆来拟合血管截面,使得重建血管的更加接近真实形态。 本文研究了冠状动脉造影定量分析,这包括二维定量分析和三维定量分析。在二维造影图上计算了病变血管狭窄程度,在三维重建的基础上计算血管长度、分支夹角、体积等参数,通过实验证明了三维定量分析的准确性和优势,实现了一种基于三维重建结果选取感兴趣血管段近似最佳投影角度的方法。利用可视化开发工具VTK,实现了交互式定量分析模块。 在造影图序列重建的基础上定性和定量分析了冠状动脉的运动特点。提出了对重建序列求局部运动的方法,以丰富冠状动脉模型运动信息。
[Abstract]:Coronary angiography is the most effective method to detect cardiovascular disease, but its two-dimensional imaging method increases the difficulty of reading film and restricts the accuracy of quantitative analysis.Three-dimensional reconstruction provides a new solution for clinical diagnosis and treatment. It can provide visual and real coronary artery spatial structure, facilitate doctors to locate lesions and accurately measure vascular parameters.The reconstruction of angiography sequence can also fully mine the information of angiogram, estimate the parameters of blood vessel motion, and study the cardiac movement.In this paper, the principle of 3D reconstruction with two angles of view is analyzed, the factors influencing the reconstruction accuracy are analyzed, and an optimization method is proposed to satisfy the reconstruction results of subsequent analysis.In order to better express the reconstructed blood vessel surface, this paper extends the general cylindrical model which describes the cross section of blood vessel by circle, and uses the information of two angiograms which participate in the reconstruction to fit the cross section of blood vessel with ellipse.Make the reconstruction of the blood vessel closer to the true form.The quantitative analysis of coronary arteriography was studied in this paper, including two-dimensional quantitative analysis and three-dimensional quantitative analysis.The degree of vessel stenosis was calculated on two-dimensional angiography, and the parameters of vascular length, angle of branch and volume were calculated on the basis of 3D reconstruction. The accuracy and advantage of 3D quantitative analysis were proved by experiments.A method to select the approximate optimal projection angle of the interested vascular segment based on the 3D reconstruction results is implemented.The interactive quantitative analysis module is realized by using visual development tool VTK.The characteristics of coronary artery motion were qualitatively and quantitatively analyzed on the basis of sequence reconstruction of angiogram.In order to enrich the motion information of coronary artery model, a method of finding local motion for reconstruction sequence is proposed.
