MSCT结合仿真内镜与电子结肠镜对结直肠癌诊断的对比研究

【摘要】:目的: 了解MSCT结合仿真内镜对结直肠癌的诊断价值。 材料与方法: 收集2010年10月-2011年11月45例结直肠疾病患者的临床及影像学资料,男27例,女18例,年龄21-74岁,平均年龄47.5岁。45例患者行电子结肠镜、CT结肠仿真内镜检查及手术病理证实。全部患者均行肠道准备及肠腔内注气后用Definition AS+128层螺旋CT进行扫描。其中32例行CT平扫加增强扫描,13例行CT平扫。平扫时扫描体位为仰卧位及俯卧位各一次。图像重建层厚1mm。应用后处理工作站colon软件进行图像后处理,评价病变的部位、形态、大小、侵犯范围、有无其它器官及淋巴结转移。 结果: 45例患者中,病理证实结、直肠癌42例,,炎症3例。42例肿瘤性病变(20例结肠癌、22例直肠癌),其中肿块型17例、浸润型10例、溃疡型6例、混合型9例。4例发现肝脏转移、39例淋巴结转移。CT结肠仿真内镜与电子结肠镜对结肠肿瘤的检出、定性及定位比较的差异P>0.05,无统计学意义。 结论: 多层螺旋CT结合仿真内镜对结直肠肿瘤的检出、定位、定性的准确率较高;对观察病灶周围的侵犯情况及有无远处器官和淋巴结转移的情况有很大的优势,同时还可以了解肠腔内病变的形态及病变累及肠腔的长度。
[Abstract]:Objective: To investigate the diagnostic value of MSCT combined with virtual endoscopy in colorectal cancer. Materials and methods: The clinical and imaging data of 45 patients with colorectal diseases from October 2010 to November 2011 were collected. There were 27 males and 18 females aged 21-74 years, with an average age of 47.5 years. All patients were scanned with Definition as 128 slice spiral CT after intestinal preparation and intraluminal gas injection. In 32 cases, CT plain scan and enhanced CT scan were performed in 13 cases. The scanning position was in supine position and prone position respectively. The thickness of image reconstruction layer is 1 mm. The post processing workstation colon software was used to evaluate the location, shape, size, invasion range, other organs and lymph node metastasis. Results: Among the 45 cases, 42 cases were confirmed by pathology, 42 cases were inflammation, 20 cases were carcinoma of colon cancer, 17 cases were mass type, 10 cases were infiltrating type, 6 cases were ulcer type. Liver metastasis was found in 39 cases of mixed type. Ct virtual endoscopy and electronic colonoscopy were used to detect colon tumor (P > 0.05), and there was no significant difference between qualitative and localization (P > 0.05). Conclusion: Multi-slice spiral CT combined with virtual endoscopy has a high accuracy in detecting, locating and determining colorectal tumors, and has great advantages in observing the invasion around the lesion and the presence of distant organs and lymph node metastasis. At the same time, we can understand the shape of the lesions and the length of the lesions involved in the lumen.
