宝石CT低剂量评价肺功能的可行性初探

【摘要】:目的: 探讨宝石能谱CT低照射剂量评价肺功能的可行性。 材料与方法: 随机抽取2014年1月1日至2014年3月20于我院体检人员60名,分为正常照射剂量组(NI=11)及低照射剂量组(NI=18)。正常照射剂量组受检者30例,其中男20例,女10例,年龄范围20~61岁,平均年龄40.8±10.8岁;低照射剂量组(NI=18)受检者30例,其中男性16例,女性14例,年龄范围24~62岁,平均年龄为44.8±11.1岁。所有受检者行宝石能谱CT胸部吸气末屏气扫描,利用后处理软件ThoracicVCAR分析图像数据并测量右肺上叶尖段气道壁平均厚度(AverageWall Thickness,WTAvg)、管径厚度与外径比值(Wall Thickness/AirwayDiameter,WT/D)、气道壁面积与气道总面积比值(Wall AreaRatio,WAR)、气道壁面积(Wall area,WA);测量出双肺各CT值区域(分为-1024~-910Hu,-910~-800Hu,-800~-700Hu,-700Hu)肺像素指数(pixel index,,PI),评估两组各测量参数间有无统计学差异。记录两组受检者CT容积剂量指数(CTDIvol)、剂量长度乘积(DLP)并计算有效剂量(ED)。 结果: 利用spss19.0软件分析实验数据,经配对T检验后得出:两组受检者右肺上叶尖段气道参数WTAvg、WT/D、WAR、WA间p值分别为0.065、0.559、0.585、0.131;两组受检者双肺各区域像素指数PI-1024~-910Hu、PI-910~-800Hu、PI-800~-700Hu、PI-700Hu间P值分别为0.262、0.817、0.542、0.889。两组各项数据间均无统计学差异(p0.05)。正常剂量组、低剂量组ED分别为4.99±1.37、1.92±0.68,低剂量组ED较正常剂量组降低61.52%。 结论: 宝石CT低照射剂量(NI=18)评价肺功能可行。
[Abstract]:Objective: Objective: to evaluate the feasibility of low dose gemstone CT in evaluating pulmonary function. Materials and methods: From January 1, 2014 to March 20, 2014, 60 medical examiners in our hospital were randomly divided into normal irradiation dose group and low dose group. 30 patients (male 20, female 10, mean age 40.8 卤10.8) in the normal radiation dose group, and the low dose group (16 males and 14 females, with an average age of 44.8 卤11.1 years). All subjects were scanned by gemstone energy dispersive computed tomography (EDS) at the end of inspiratory breath holding. Using post-processing software ThoracicVCAR to analyze the image data and to measure the mean thickness of the airway wall in the upper apical segment of the right lung (AverageWall Thicknessy WTAvgn), the ratio of diameter thickness to outer diameter, Wall thickness to outer diameter ratio, Wall area to total area of the airway, Wall area of the upper lobe of the right lung, and the area of the airway wall to measure the area of the two lungs, and to measure the ratio of the wall area to the total area of the airway. The pixel index of each CT value region (divided into -1024 ~ 910Hu-910Hu-800 Hu-800 Hu-800 Hu-700 Hu-700 Hu-700 Hu-7H) lung pixel index was used to evaluate the statistical difference between the two groups. The CT volumetric dose index (CTDIvoll), dose length product (DLPP) and effective dose (EDV) were recorded in the two groups. Results: Spss19.0 software was used to analyze the experimental data. After paired T test, the following results were obtained: the airway parameters of the right upper pulmonary apical segment in the two groups were as follows: the value of the WTAvgGV WTP / WARWA was 0.065 0. 559 0. 585% 0.131, and the pixel index of each region of the lung was PI-1024-910Hu-910Hu-800 Hu-PI-800 Hu-700 Hu value of 0.2620.817U 0.5420.80.88898.The two groups were divided into two groups, and the results showed that the mean values of the two groups were 0.2620.817 and 0.5420.8889, respectively, and the pixel index of the two groups was PI-1024-910Hu-910Hu-800 Hu-PI-800, PI-700Hu. There was no statistical difference between the two groups (p 0.05). Ed in normal dose group and low dose group were 4.99 卤1.37 and 1.92 卤0.68 respectively. Ed in low dose group was 61.5222lower than that in normal dose group. Conclusion: The evaluation of lung function by low dose sapphire CT was feasible.
