鼓室成形术与CT三维重建测量研究

【摘要】:目的:1、利用多排螺旋CT多平面重建、mimics听骨链三维重建以及镫骨前后弓斜位重建测量中耳正常解剖结构相关数据。2、探讨鼓室成形术中人工听骨假体的个体化精确塑形以及植入的PORP与鼓膜及镫骨的角度关系。 材料与方法:1、成年国人无耳疾颞骨33只。均为临床无中耳症状、CT检查未见中耳异常。 2、所有病例均行颞骨16排螺旋CT扫描检查。患者仰卧位,平行于听眦线自外耳道下缘至岩骨上缘做轴位CT扫描,层厚0.75mmm,螺距1.0,电压120KV,电流187mAs,扫描时间约4.5-5.5s,视野:107mm。常规颞骨冠、轴位重建并摄片,将数据重建图像传至工作站。 3、后处理:所有33耳均行MDCT多平面重建,以骨算法平行于砧骨豆状突、通过卵圆窗的角度对镫骨前后脚进行斜轴位重建。mimics10.01软件行听骨链三维重建。 4、计算:运用数学公式进行计算,算出镫骨前后弓平面与鼓膜平面的夹角平均值及标准差。 结果 MDCT的MPR及镫骨前后脚斜轴位重建法可清楚显示三块听小骨的位置、形态。 mimics10.01软件重建的三维图像可立体显示各结构之间的空间关系,计算获得 镫骨前后弓平面与鼓膜平面的夹角为47.4±12.0。 结论 本实验利用影像学方法和计算机软件计算出正常国人颞骨中鼓膜平面与镫骨前后弓平面所成夹角为47.4±12.0。。对听骨链重建术中人工听骨的个性化精确塑形、术中人工听骨假体的摆放角度(主要是指PORP)以及新型人工听骨假体的开发研究具有一定价值。
[Abstract]:Objective to investigate the individualized and accurate reconstruction of artificial ossicular prosthesis in tympanoplasty by using multislice spiral CT multiplanar reconstruction of mimics auditory chain 3D reconstruction and reconstruction of anterior and posterior stapes arch oblique position to measure the normal anatomical structure of middle ear. The shape and the angle of the implanted PORP to the tympanic membrane and stapes. Materials and methods: 1, 33 adult Chinese temporal bone without ear disease. All of them had no symptoms of middle ear and no abnormality of middle ear on CT examination. 2. All patients underwent 16-slice spiral CT scan of temporal bone. The patient was supine and performed axial CT scan parallel to the line of canthus from the lower edge of the external auditory canal to the superior edge of the petrosal bone. The slice thickness was 0.75 mm, pitch 1.0, voltage 120kV, current 187mAs. the scanning time was about 4.5-5.5 s, and the visual field was: 107mm. Routine temporal bone crown, axial reconstruction and radiography, data reconstruction image to the workstation. 3, postprocessing: all 33 ears underwent MDCT multiplanar reconstruction, the bone algorithm was parallel to the lenticular process of the incus, and the oblique axis reconstruction of the anterior and posterior stapes was performed by the angle of the oval window. The auditory chain was reconstructed by the software .mimics10.01. 4, calculation: the average angle and standard deviation between the anterior and posterior arch plane and tympanic membrane plane of stapes were calculated by mathematical formula. Result MDCT's MPR and anterior and posterior stapes oblique axis reconstruction can clearly display the position and shape of the three auditory ossicles. The 3D image reconstructed by mimics10.01 software can show the spatial relationship between the structures stereoscopically. The angle between the anterior and posterior arch plane of stapes and the tympanic plane was 47.4 卤12.0. Conclusion The angle between the tympanic membrane plane and the anterior and posterior arch plane of the stapes was calculated by using the imaging method and computer software in the normal Chinese temporal bone. The angle between the tympanic membrane plane and the anterior arch plane of the stapes was 47.4 卤12.0.. It is of value to the development of the artificial ossicular prosthesis in the reconstruction of ossicular chain, the angle of placement of artificial ossicular prosthesis (mainly referred to as PORP) and the development of new artificial ossicular prosthesis.
