山西省武警战士非特异性腰痛现况调查及其对生存质量影响研究

【摘要】:目的:探讨武警战士这一特定人群执勤与训练期间非特异性腰痛(nonspecific low back pain,NLBP)的患病率、危险因素及其对生存质量的影响,为改善武警部队官兵勤务及训练条件以预防腰背部训练伤和提高生存质量提供理论依据。方法:2016年9~12月,随机整群抽取驻山西武警某部战士共1385名。制订专用《腰痛调查表》,包括调查对象的基本信息、腰痛部位、时间和程度、执勤和训练情况、VAS疼痛视觉评分和Oswestry功能障碍指数评估(ODI),生存质量测评采用世界卫生组织生存质量测定简表(WHOQOL-BREF)。采用非条件Logistic回归分析腰痛影响因素,生存质量影响因素分析采用多重线性回归。结果:本次调查共发放问卷1385份,收回有效问卷1327份,应答率为95.8%。武警战士非特异性腰痛整体患病率为36.4%。进入与训练相关腰痛回归模型的危险因素有腰部外伤史(Odds Ratio,OR=4.477,95%CI:3.081-6.505)、训练压力(OR=1.937,95%CI:1.502-1.497)、军龄(OR=1.343,95%CI:1.697-3.235)、训练满意度(OR=0.470,95%CI:0.243-0.908)等。腰痛战士总的生存质量及各领域的生存质量均明显低于非腰痛者(P0.001)。训练满意度与武警战士的生存质量呈正相关,腰外伤史、训练压力、每天站立时间与武警战士的生存质量呈负相关。结论:非特异性腰痛会影响武警战士的生存质量,致腰痛的高危因素,如腰外伤史、训练压力、训练满意度、长时间站岗等直接或间接导致腰痛患者生存质量下降。各级指挥员应合理安排训练内容和强度,以降低武警战士非特异性腰痛的发生,进一步改善其生存质量。
[Abstract]:Objective: to investigate the prevalence, risk factors and effects of nonspecific low back pain (NLP) on the quality of life (QOL) of armed police soldiers during their duty and training. It provides a theoretical basis for improving the duty and training conditions of the officers and soldiers of the armed police force in order to prevent the injury of the back and waist and improve the quality of life. Methods: from September to December 2016, 1385 soldiers were randomly selected from a unit of Shanxi Armed Police Force. Develop a special "low back pain questionnaire", including basic information on the subjects, location, time and extent of low back pain, VAS pain visual score and Oswestry dysfunction index (Oswestry dysfunction index) were evaluated on duty and training. The quality of life was evaluated by World Health Organization (WHO) summary table of quality of life (WHOQOL-BREFFN). Non-conditional logistic regression analysis was used to analyze the influencing factors of low back pain and multiple linear regression was used to analyze the influencing factors of quality of life. Results: 1385 questionnaires were sent out, 1327 valid questionnaires were collected, and the response rate was 95.8%. The overall prevalence of non-specific low back pain in armed police soldiers was 36. 4%. The risk factors for entering the regression model of low back pain related to training were as follows: the history of waist trauma: Odds Ratiol: 4.477 / 95CII: 3.081-6.505; training stress: 1.937 / 95CIW 1.502-1.497; military experience: 1.34395CIW 1.697-3.235; training satisfaction: 0.470,95CIW 0.243-0.908). The total quality of life (QOL) and quality of life (QOL) of low back pain soldiers were significantly lower than those of non-low back pain patients (P 0. 001). The satisfaction degree of training was positively correlated with the quality of life of armed police soldiers. The history of waist trauma, training pressure and standing time each day were negatively correlated with the quality of life of armed police soldiers. Conclusion: Non-specific low back pain can affect the quality of life of armed police soldiers, and the high risk factors of low back pain, such as history of waist trauma, training pressure, training satisfaction, long time standing on guard, etc, directly or indirectly lead to the decline of the quality of life of patients with low back pain. Commanders at all levels should arrange the training content and intensity reasonably in order to reduce the incidence of non-specific low back pain and further improve the quality of life of armed police soldiers.
