基于EPID的容积旋转调强放疗绝对剂量验证探讨

【摘要】:目的:探讨电子射野影像装置(electronic portal imaging device,EPID)用于容积旋转调强放疗(volumetric modulated arc therapy,VMAT)绝对剂量验证的可行性与在临床应用中的意义。方法 :随机选取100例患者的VMAT治疗计划,用Eclipse软件导出EPID计划和水模体验证计划等中心平面的通量图。在2幅通量图中对应位置处,各随机选取30个待测量绝对剂量点(靶区内6个,靶区外扩5、10、20、30、50 mm的区域内和有效探测范围内各4个点),按照选取位置的不同将这些剂量点分成7组,每个点均测量EPID和指形电离室实测值,用单因素F检验比较2种方法7组数据的计划值与实测值准确率的差异。结果 :7组剂量点的绝对剂量验证结果准确率基本均在98%以上,2种方法测量的7组数据的准确率均无统计学差异(F=0.032-3.189,P0.05)。结论:EPID具有在线实时剂量验证的潜力和明显优势,能够大大提高多点绝对剂量验证的效率,可以用于VMAT治疗绝对剂量的验证。
[Abstract]:Objective: to investigate the feasibility and clinical significance of (electronic portal imaging devicereal pid (EPID) in absolute dose verification of (volumetric modulated arc therapeutic VMAT. Methods: 100 patients with VMAT were randomly selected and the flux maps of EPID plan and water model experience plan were derived by Eclipse software. At the corresponding position in two flux maps, 30 absolute dose points to be measured were randomly selected (6 in target region, 6 in target region). These dose points were divided into 7 groups according to the selected position. EPID and finger ionization chamber were measured at each point. A single factor F test was used to compare the difference between the planned values and the actual values of 7 groups of data of the two methods. Results there was no significant difference in the accuracy of absolute dose verification between the 7 groups (F0. 032-3. 189). The accuracy rate of absolute dose verification was above 98% (F0. 032-3. 189) in the 7 groups (F0. 032-3. 189). Conclusion: EPID has the potential and obvious advantage of on-line real-time dose verification. It can greatly improve the efficiency of multi-point absolute dose verification and can be used to verify the absolute dose of VMAT.
