动态增强MRI与乳腺X线摄影在乳腺病变诊断中的应用对比

【摘要】:目的探讨MRI动态增强扫描(dynamic contrast-enhancement magnetic resonance imaging,DCE-MRI)与乳腺X线摄影在乳腺病变诊断中的应用效果。方法对在我院收治的乳腺病变患者应用DCE-MRI与乳腺X线摄影进行检查,并就这两种检查方法在发现病变及良恶性肿块鉴别方面的诊断能力进行对比研究,以期客观评价两种检查方法对乳腺疾病的诊断效能。结果以病理确诊结果为金标准,计算乳腺X线摄影诊断良、恶性乳腺病变的敏感性为65.63%,特异性为70.37%,准确性为68.60%;DCE-MRI诊断良、恶性乳腺病变的敏感性为87.50%,特异性为90.74%,准确性为89.53%;经统计学分析发现DCE-MRI诊断良、恶性乳腺病变的敏感性、特异性及准确性均明显高于乳腺X线摄影,且差异具有统计学意义。结论乳腺X线摄影和动态增强MRI对乳腺病变的鉴别诊断均有一定的实用价值,但是乳腺动态增强成像能够提供更加丰富的形态学信息,诊断效能优于乳腺X线摄影,两者的结合可以发挥优势互补的作用。
[Abstract]:Objective to evaluate the effect of MRI dynamic enhanced scanning (DCE-MRI) and mammography in the diagnosis of breast lesions. Methods DCE-MRI and mammography were performed in patients with breast lesions in our hospital, and the diagnostic ability of the two methods in the differential diagnosis of benign and malignant masses was compared. Objective to evaluate the effectiveness of the two methods in the diagnosis of breast diseases. Results according to the gold standard of pathological diagnosis, the sensitivity, specificity and accuracy of DCE-MRI were 65.63, 70.37 and 68.60m respectively. The sensitivity, specificity and accuracy of DCE-MRI in the diagnosis of benign and malignant breast lesions were 87.50%, 90.74 and 89.53, respectively. The sensitivity, specificity and accuracy of DCE-MRI in the diagnosis of benign and malignant breast lesions were significantly higher than those in mammography, and the difference was statistically significant. Conclusion both mammography and dynamic contrast enhanced MRI have some practical value in the differential diagnosis of breast lesions, but dynamic enhanced mammography can provide more abundant morphological information, and the diagnostic efficacy is better than that of mammography. The combination of the two can play a complementary role.
