低剂量CT扫描检测肺结节的实验研究

【摘要】:目的评价能谱CT胸部低剂量扫描的图像质量和对肺结节检出的敏感性,探讨最佳的扫描参数。 方法制作三组不同大小的软组织密度的小结节(直径分别为10mm、5mm、3mm),置入充气膨胀的猪肺中,使用能谱CT分别行常规剂量扫描(管电压120kVp,螺距0.984:1,管电流150mA)与低剂量扫描(管电压120kVp,螺距0.984:1,管电流50mA、30mA、20mA、10mA),记录不同扫描方式的辐射剂量,评价图像的噪声与伪影情况,评估低剂量扫描对肺结节检出的敏感性。 结果本研究胸部低剂量扫描的辐射剂量仅为常规剂量的6.7%—33.3%。低剂量扫描测得图像的CT值与常规剂量相比无明显差异(P0.05),但是CT值标准差的差异有统计学意义(P0.05)。仅在20mA和lOmA扫描的图像中出现条形伪影。50mA扫描与常规剂量扫描一致性较好(0.4≤kappa0.75);20mA扫描能清晰显示肺支气管Ⅲ级以上分支,并能检出所有结节;lOmA扫描会遗漏直径3mm的结节。 结论能谱CT胸部低剂量扫描的辐射剂量明显降低;降低管电流虽然使图像噪声和伪影增大,但是仍然可以较好的显示肺内结构和病灶;实验条件下20mA可作为检测肺结节的可选剂量。
[Abstract]:Objective to evaluate the image quality of low dose CT chest scanning and its sensitivity to the detection of pulmonary nodules and to explore the best scanning parameters. Methods three groups of small nodules (10 mm to 5 mm / 3 mm in diameter) with different sizes of soft tissue densities were made and placed into inflatable pig lungs. Energy dispersive CT was used for routine dose scanning (tube voltage 120kVp, pitch 0.984: 1, tube current 150mA) and low-dose scanning (tube voltage 120kVp, pitch 0.984: 1, tube current 50mAn 30mA10 Ma). Radiation doses of different scanning methods were recorded, and the noise and artifacts of the images were evaluated. To evaluate the sensitivity of low dose scanning to the detection of pulmonary nodules. Results the radiation dose of low-dose chest scan was only 6.7-33. 3% of the conventional dose. There was no significant difference in CT value between low dose scan and conventional dose (P0.05), but the difference of standard deviation of CT value was statistically significant (P0.05).
