迭代算法改善不同BMI患者双源CT腹部低剂量扫描图像质量的研究

【摘要】:目的:探讨迭代重建算法(SAFIRE)对在通过降低管电压来实现的腹部低剂量扫描患者图像质量的改善情况和剂量的降低程度。材料和方法:将294例腹部增强扫描患者(男174例、女120)根据随机原则按扫描管电压的高低(120KV扫描、100KV扫描)和BMI指数(标准体重18.5 kg/m2≤BMI24 kg/m2、超重24 kg/m2≤BMI28 kg/m2)分成四组:A组(120KV扫描、标准体重)、B组(100KV扫描、标准体重)、C组(120KV扫描、超重)、D组(100KV扫描、超重)。记录四组的辐射剂量CTDIvol、DLP,计算ED。扫描后四组动脉期和静脉期均通过滤波反投影(FBP)法重建图像,B组、D组增加迭代(SAFIRE)法重建图像。对所有图像由两位高年资影像诊断医生进行评分(1—5分),测量所有病人动脉期和静脉期肝门层面肝脏、竖脊肌和动脉期腹主动脉、静脉期门静脉的CT值和标准差,测量皮下脂肪的标准差作为图像噪声。计算SNR和CNR,计算公式为SNR脏器=ROI脏器/SD脏器;CNR脏器=(ROI脏器-ROI肌肉)/SD脂肪。比较A组和B组、C组和D组的辐射剂量ED,将A组、B组和C组、D组FBP重建的图像进行独立样本比较,将B组和D组FBP重建图像和SAFIRE重建图像进行配对样本比较,比较内容为:动脉期和静脉期的主观评分、图像噪声、腹主动脉、门静脉、肝脏的SNR和CNR。结果:辐射剂量:B组、D组的CTDIvol、DLP、ED明显低于A组、C组。主观评价:在动脉期FBP重建和静脉期FBP重建图像,A组、B组无差异;C组优于D组;B组、D组无论在动脉期还是静脉期,其SAFIRE重建图像优于FBP重建图像。客观评价:在FBP重建时B组、D组的图像噪声高于A组、C组;B组、D组使用SAFIRE重建图像噪声低于使用FBP重建图像噪声。FBP重建动脉期图像的A、B组的SNR腹主动脉以及C、D组的CNR腹主动脉、CNR肝脏无统计学差异;A组的SNR肝脏优于B组,B组CNR腹主动脉、CNR肝脏的优于A组,C组的SNR腹主动脉、SNR肝脏优于D组。静脉期图像A、B组的SNR门静脉,C、D组CNR门静脉、CNR肝脏无统计学差异;A组的SNR肝脏优于B组,B组的CNR门静脉、CNR肝脏优于A组,C组的SNR门静脉、SNR肝脏优于D组。B组、D组SAFIRE重建图像的信噪比和对比噪声比无论动脉期还是静脉期均明显优于FBP重建。结论:标准体重患者和超重患者腹部增强CT检查时都可以通过降低管电压来获得低剂量扫描;超重患者降低管电压在常规方式重建所得的图像质量会下降,可采用迭代重建算法(SAFIRE)加以弥补。
