坐位下体负压对人体生理心理功能影响的实验研究

【摘要】: 飞行中飞行员可能暴露于+Gz环境下,+Gz负荷引起脑血流量减少到临界值时,所发生的意识突然丧失的状态称为G致意识丧失(G-LOC)。近年来,随着高性能战斗机的发展,高G值(>9G),快增长率(>6G/s)的持续性正加速度的医学防护,特别是高过载致空中意识丧失的预防和监控,对降低G-LOC的发生率,预防飞行事故的发生,提高飞行部队的战斗力,仍是航空航天医学研究的重要课题之一。 下体负压(LBNP)是模拟重力-惯性力场对人体循环功能影响的有效研究方法之一,在一定程度上可模拟正加速度对人体的影响。鉴于未来高技术战争的需要和飞行部队的实际,有必要探讨下体负压对人体脑血流和脑认知功能的影响,以此作为检测G-LOC的手段和方法。 本实验的目的在于:1)观察坐位下体负压条件下人体大脑中动脉血流速度及耳部血氧饱和度的变化;2)探讨坐位下体负压对人体脑事件相关电 第四草医大学硕士学位论文 一 位爬RPX 反应时(RTX跟踪误差oR)等大脑认知功能的影响,为空中晕厥和 立位耐力不良的医学鉴定提供人体实验依据。 本研究选取13叫 名青年男性作为被试者。每名被试者均要在两种坐 位下体负压条件下各进行两个实验。两种负压条件分别为:t.00 kPa(-30 InmHg)和-6.67 kPa(50 mlnHg卜每个实验要在每种负压条件下各测试 5 min,以负压前 5 min测试的指标作为对照。 实验一,15名被试者,主要观察坐位下体负压条件下对人体大脑中动 脉血流速度(V。c。L血氧饱和度(SaO*和心率(HR)的影响。 实验二,13名被试者,主要观察坐位下体负压条件下视觉 Oddball模 式诱发的脑事件相关电位、双任务反应时、正确率aR郴跟踪误差的变化c 同时进行血压(BP)和心率的检测。 两个实验均记录被试者的主观症状和客观体征。其上要实验结果如下: 1.坐位下体负压对人体大脑中动脉血流速度、心率和血氧饱和度的影 响。在-4.00 kPa下体负压作用4、5 min时,脑血流速度减慢(尸<0.05);负 压作用3、4、smln时。0率增快沪<o5);血氧饱和度无明显改变。在巧石7 Wa下体负压作用2、3 min时,脑血流速度减慢什<几05) 在4和5 min时 显著减慢o<0.of),卸压后 lmin尚未恢复,随后恢复至对照水平;负压作 用后各时间点。乙率均显著增快沪叩刀1\ 血氧饱和度在负k暴露5加D时明 显下降(尸<0刀5)。 2.坐位下体负压对脑认知工效及心血管功能的影响。在4刀0 kPa下 体负压作用时,Psoo波幅(AMP)、Psoo潜时(LAT)、双任务反应时、跟踪 误差、血压与负压前对照无明显差别:心率在下体负压作用3、4、5叫n 时,显著增快什<0刀5卜在6石7wa下体负压作用时,马皿波幅显著降低 (<0.05),p300潜时显著延长(p<0.05);反应时延长(<0.05),足踪误差增大 沪刃仍):心率在负压作用各时问点与负压作用前比较均显著增快什<o01); .二l- 第四罕医大学硕士学位论文 一 收缩压与负压前对照无明显改变,舒张压显著升高(P<0.05)。 实验结果表明,坐位下体负压造成大脑中动脉血流速度减慢,引起大 脑供血量减少,血液在下肢积聚,大脑对外界刺激的反应减慢,脑认知工 效下降,同时心血管部分代偿功能加强,心率增快,,血压升高,为空中晕 厥和立位耐力不良的医学鉴定提供了人体实验依据。
[Abstract]:The pilot in flight may be exposed to the +Gz environment, +Gz load caused by cerebral blood flow is reduced to a critical value, the sudden loss of consciousness state called G induced loss of consciousness (G-LOC). In recent years, with the development of high performance fighter, high G value (> 9G), fast growth rate (> 6G/s) medical protection sustained positive acceleration, especially high overload caused by the prevention and monitor of unconsciousness, to reduce the incidence of G-LOC, prevention of flight accidents, improve flight fighting force, is still an important subject of aerospace medicine.
