湿热环境下创伤兔应用低温输液疗法的可行性研究

【摘要】:本课题来源:总后勤部“十五”攻关课题资助项目No.01Z100。总课题名称:特殊环境下战创伤护理技术与装备研究。 背景:我军把长江以南的东南沿海省域,如浙江、福建、广东、广西、海南、台湾以及江苏南部、云南南部和西南部海拔在1500m以下的谷地划为热带地区,属湿热气候。这一特殊环境下,创伤感染率高,应激刺激强度大,机体损伤程度重。随着新时期国际局势的变化及我军战略重点的转移,高温高湿环境下战创伤的救治与护理成为我军医疗护理工作的重点。此外,我们所处的广东地区长夏暖冬,夏季炎热多湿,太阳辐射强,平均气温28℃,极端气温38℃~41℃,相对湿度可达85%~98%。尤其是南海诸岛,四周环海,处于典型的湿热海洋气候,空气中的相对湿度高达100%,常年气温30℃以上多达8个月,而且本地区经济发达,工伤交通事故多。因此,加强高温高湿环境下战创伤的救护技术研究有重要的军事和社会意义。 高温、高湿复合因素不仅增加了机体的代谢消耗,较易发生内环境紊乱,还降低了机体在创伤后的应激和抵抗能力,使战创伤的伤情更加严重和复杂。另外,由于高温造成体内水分大量丢失,在伤后失血的基础上,更加重了循环血容量的不足,造成全身各脏器灌注不足。因此,有必要及时给予补液和有效降温措施。而目前现有的降温措施存在着降温效果不稳定(如酒精擦浴、冰帽)、体积大,不易携带(如降温机)或是不易操作(如体外血液冷却法、腹腔低温
