乳酸林格液复苏对弹烧复合伤后海水浸泡血浆电解质及渗透压的影响

【摘要】:目的研究乳酸林格液复苏对弹烧复合伤海水浸泡动物血浆电解质及渗透压的影响,为选择复苏用液体提供理论依据。方法成功建立弹烧复合伤海水浸泡动物模型后,将14只成年杂种犬随机分为弹烧复合伤海水浸泡组(简称浸泡组,n=8只)和弹烧复合伤海水浸泡乳酸林格液复苏组(简称复苏组,n=6只),动态监测伤前、伤后4,7,10,20,28h共6个时相点外周动脉血钠、钾、氯及血浆总渗透压的变化。结果弹烧复合伤后海水浸泡组动物血浆电解质及渗透压上升非常明显,并且贯穿在整个实验过程;而采用乳酸林格液复苏后,动物血浆电解质及渗透压明显较浸泡组下降。结论乳酸林格液能纠正弹烧复合伤海水浸泡所出现严重的高钠、高氯、高渗血症。
[Abstract]:Objective to study the effect of Ringer lactate resuscitation on plasma electrolytes and osmotic pressure in marinated animals after combined bullet burn injury and to provide theoretical basis for the selection of fluid for resuscitation.Methods the animal model of projectile burn combined injury in seawater immersion was established successfully.Fourteen adult mongrel dogs were randomly divided into two groups: gunfire combined with seawater immersion group (8 dogs) and elastic burn combined injury group with lactate Ringer solution resuscitation group (6 dogs), and dynamic monitoring before injury.Changes of peripheral arterial blood sodium, potassium, chlorine and plasma total osmotic pressure at 6 time points (4: 7, 10: 20, 20: 28 h after injury).Results the plasma electrolyte and osmotic pressure in the sea water immersion group were significantly increased after the combined injury, and the plasma electrolyte and osmotic pressure were significantly decreased after the resuscitation of Ringer lactate solution in the sea water immersion group, and the plasma electrolyte and osmotic pressure in the seawater immersion group were significantly lower than those in the immersion group.Conclusion Ringer lactate solution can correct the severe hypernatremia, hyperchlorination and hyperosmosis in seawater immersion caused by bomb burn combined injury.
