中西医结合治疗老年抑郁之研究

【摘要】:随着人口平均寿命显著延长,世界上许多国家与我国对老年医学已极为重视。而抑郁症是二十一世纪威胁人类健康的三大卫生课题之一,联合国世界卫生组织针对全球性的疾病负担估算,指出至二○二○年,抑郁症将成为所有疾病中排名第二的高负担与失能疾病。但是流行病学的调查研究显示老年人抑郁症状的盛行率远超过抑郁疾患的盛行率。所以本课题是研究包含老年抑郁症在内的老年抑郁状态相关的流行病学研究;中医对老年抑郁的认识、病因病机的探讨、辨证分型与治疗;西医对老年抑郁的认识、病理变化的研究、诊断的标准、鉴别诊断与治疗;以及中西医结合治疗老年抑郁的现况,而目前大都是以中药或针刺配合较小剂量的西药为研究较多,降低单用西药的副作用以提高疗效。此外,中西医结合治疗老年抑郁尚须解决的问题包括:辨证分型急待规范、有效治疗药物及方剂的筛选、重视不同证型之间重叠性的研究以及制定规范统一的疗效标准。如此将有助于中西结合治疗老年抑郁的实证式研究。 西医对老年抑郁仍欠缺快速的筛检检测仪器,我们首次尝试以计算机经络能量分析仪器MEAD,探讨临床上快速筛检老年抑郁状态的可行性。我们针对70位65岁以上的老人,,研究分析经由老年抑郁量表(Geriatrics Depression Scale,GDS)评量与计算机经络能量分析仪器检测结果彼此之间的相关性。结果显示:GDS量表的评量分数与十二经脉中的肝经偏离平均值的变化程度有显著的正相关(p<0.01);而与阴阳经脉的导电量总合比值则是呈显著的负相关(p<0.05)。这结果与肝气郁结是抑郁表现的主要病机,以及脏气虚弱是老年抑郁的重要内在发病因素相谋合。由本研究结果得知,计算机经络能量分析仪器有可能成为临床上辅助快速筛选老年抑郁状态的客观检查仪器。
[Abstract]:With the increase of life expectancy, many countries in the world and our country have attached great importance to geriatrics. And depression is one of the three major health issues that threaten human health in the 21 century. The United Nations World Health Organization (WHO) estimates the global burden of disease and points out that by the year 2020, Depression will be the second highest burden and disability of all diseases. But epidemiological studies show that the prevalence of depression in the elderly is much higher than the prevalence of depression. Therefore, this topic is to study the elderly depression including the elderly depression related epidemiological study, TCM understanding of the elderly depression, etiology and pathogenesis of the discussion, syndrome differentiation and treatment; Western medicine to the elderly depression understanding, pathological changes, diagnostic criteria, differential diagnosis and treatment; And the current situation of integrated traditional Chinese medicine and western medicine in the treatment of depression in the elderly, but at present, most of them have been studied by traditional Chinese medicine or acupuncture combined with small dose of western medicine, so as to reduce the side effects of single use of western medicine in order to improve the curative effect. In addition, the problems to be solved in the treatment of senile depression by combination of traditional Chinese and western medicine include the urgent standardization of syndrome differentiation and classification, the screening of effective drugs and prescriptions, the emphasis on overlapping studies among different syndrome types, and the formulation of standardized and unified therapeutic standards. This will contribute to the combination of Chinese and Western medicine in the treatment of senile depression empirical study. There is still no rapid screening instrument for senile depression in western medicine. We first tried to use the computer meridian energy analysis instrument MEAD, to explore the feasibility of rapid screening for senile depression in clinic. We studied and analyzed the correlation between the results of the Geriatric Depression scale (Geriatrics Depression Scale,GDS) and the results of computer meridian energy analysis in 70 elderly people over 65 years old. The results showed that there was a significant positive correlation between the score of the GDS scale and the mean deviation of the hepatic meridians in the twelve meridians (p < 0 01). There was a significant negative correlation between the total electric conductivity ratio of yin-yang meridian and Yin-Yang meridian (p < 0.05). These results are consistent with the main pathogenesis of depression, and the weakness of visceral qi is an important internal pathogenic factor of depression in the elderly. According to the results of this study, the computer meridian energy analysis instrument may be used as an objective examination instrument to assist in the rapid screening of depression in the elderly.
