川东北地区初诊食管癌患者营养状况及生活质量调查分析

【摘要】:目的:了解川东北初诊食管癌患者营养状况及生活质量基本情况,分析相关影响因素,为临床诊治提供参考。方法:收集川东北地区经病理学检查确诊的初诊食管癌病例,利用人体测量指标及相关实验室指标评估营养状况,采用欧洲癌症病人生命质量核心量表(EORTC QLQ-C30 V3.0中文版)、食管癌专用量表(EORTC QLQ-OES18)评估生活质量,采用SPSS单因素及多因素分析初诊食管癌患者营养状况及生活质量的影响因素。结果:收集川东北地区经病理学检查确诊的初诊食管癌病例481例,按体质指数、血清白蛋白、血清前白蛋白、转铁蛋白、淋巴细胞总数、肱三头肌皮褶厚度作为评价指标,营养不足发生率分别为11%、10%、90.9%、84.4%、80.9%、56.5%。以体质指数为判定指标,单因素分析显示,不同C反应蛋白(C-reactive protein,CRP)水平、病变长度5cm、梗阻程度与食管癌患者营养不足有关(P0.05),Logistic回归分析显示,CRP水平及梗阻程度是食管癌患者营养不足的影响因素。初诊食管癌患者的生活质量评分较低,总体健康状况平均64.30±14.33分,功能领域得分从低到高前三位依次是社会功能、情绪功能、角色功能。单因素分析显示,存在营养不足的患者在总体健康状况、气促、腹泻、咽口水、咳嗽、言语、梗阻方面与非营养不足患者相比差异有显著性(P0.05),营养不足患者生活质量较差。结论:川东北地区初诊食管癌患者营养不足发生率较高,CRP水平及梗阻程度是影响营养不足的主要因素;其总体生活质量偏低,营养不足是生活质量较差的重要影响因素,临床应充分重视患者的营养状况。
[Abstract]:Objective: to investigate the nutritional status and quality of life (QOL) of newly diagnosed esophageal cancer patients in Northeast Sichuan, and to analyze the related factors and provide reference for clinical diagnosis and treatment.Methods: the first diagnosed esophageal cancer cases in Northeast Sichuan were collected and the nutritional status was evaluated by anthropometric and related laboratory indexes.The quality of life (QOL) was evaluated by EORTC QLQ-C30 V3.0 and EORTC QLQ-OES18. The factors affecting nutritional status and quality of life of newly diagnosed esophageal cancer patients were analyzed by SPSS single factor and multivariate analysis.Results: 481 cases of esophageal cancer diagnosed by pathology in Northeast Sichuan were collected. Body mass index (BMI), serum albumin (SA), serum prealbumin (SA), transferrin, total lymphocyte count and triceps skinfold thickness were used as evaluation indexes.The incidence of malnutrition was 11100.90% and 84.4%, respectively.The single factor analysis showed that different C-reactive protein (CRP) levels were determined by body mass index (BMI).The length of lesion was 5 cm and the degree of obstruction was related to nutritional deficiency of esophageal cancer patients. Logistic regression analysis showed that CRP level and degree of obstruction were the influencing factors of nutritional deficiency in esophageal cancer patients.The quality of life (QOL) score of newly diagnosed esophageal cancer patients was lower, the average health status of the patients was 64.30 卤14.33. The social function, emotional function and role function were the first three points in the functional field from low to high.Univariate analysis showed that there were significant differences in the overall health status, shortness of breath, diarrhea, pharyngeal saliva, cough, speech and obstruction between the patients with malnutrition and those with non-nutritional deficiency (P 0.05), and the quality of life of the patients with malnutrition was poor.Conclusion: the high level of CRP and the degree of obstruction are the main factors affecting the nutritional deficiency in the newly diagnosed esophageal cancer patients in Northeast Sichuan, and the overall quality of life is low, and the deficiency of nutrition is the important factor affecting the poor quality of life.The nutritional status of patients should be paid more attention to in clinic.
