外周血端粒长度与非小细胞肺癌临床特征的相关性分析

【摘要】:目的:比较非小细胞肺癌患者与正常体检人群外周血相对端粒长度的差异,同时观察端粒长度与非小细胞肺癌临床特征的相关性。方法:选取2016年3月至2016年10月期间就诊于兰州军区兰州总医院,经肺部穿刺活检或气管镜下活检,病理结果显示为非小细胞肺癌(腺癌、鳞癌、大细胞癌等),共70例患者,并选取同期体检中心健康体检者40例作为对照组,检测非小细胞肺癌组及对照组外周血相对端粒长度的水平,同时统计非小细胞肺癌组的临床分期、病理类型、Ki-67阳性细胞数表达水平等,运用SPSS19.0统计学软件进行相关指标分析。结果:1.非小细胞肺癌组患者外周血相对端粒长度明显高于健康对照组(P0.05)。2.非小细胞肺癌组外周血相对端粒长度与临床分期相关,分期越晚(Ⅲ+Ⅳ),端粒长度缩小越明显(P0.05)。3.不同病理类型的非小细胞肺癌患者端粒长度不同,腺癌组较鳞癌组,端粒缩短更明显(P0.05)。4.外周血端粒长度受病灶大小的影响,病灶越大,端粒缩小越明显(P0.05)。5.非小细胞肺癌组中,淋巴结是否转移,端粒长度的变化无明显差异(P0.05)。6.非小细胞肺癌组中,是否已出现远处转移对端粒长度的变化无影响(P0.05)。7.非小细胞肺癌组中,不同Ki-67阳性细胞数表达水平与端粒长度有明显差异,Ki-67阳性细胞数表达水平越低,相对端粒长度越短(P0.05)。结论:1.非小细胞肺癌组的端粒长度较健康对照组明显缩短,有望成为非小细胞肺癌早期诊断的生物标志物之一;2.端粒长度与非小细胞肺癌的临床分期、病理类型、恶性程度密切相关,有望作为新的肿瘤标记物或药物治疗靶点。
