古代中医正骨推拿文献理论体系框架结构研究

【摘要】：目的：从正骨推拿现代研究状况,理论框架的现代研究状况和术语学相关研究现状三个部分为正骨理论框架构建的方法提供参考,分析中医正骨推拿理论框架构建的可行性和必要性。方法：通过搜索从先秦到民国时期文献收录记载有古代推拿正骨理论和(或)操作方法的书籍,其中纸质书籍以辽宁中医药大学图书馆现存文献书籍为主,电子文献以中华医典所收录的文献为主。经检索确认8本纸质文献,分别是：《肘后备急方》《诸病源候论》《千金翼方》《仙授理伤续断秘方》《医说》《圣济总录》《世医得效方》《医宗金鉴》,摘录其中与正骨推拿相关的语段输入计算机备用,尽可能保持文献原貌。记录语段相关信息,包括书名、作者、成书年代、章节、版次等。确定分词原则,对结果进行筛选,采用基于理解的分词方法对所载录的文献进行分词；对词的意向进行标注,方便归类；对相同词语且语义完全一致的词或词组进行合并,对词语中被确定为无意义虚词的词或词组进行剔除。对于以构建的树形结构进行词语填充,检查理论结构框架是否合理,能否兼容所有词语和词组(已有的词汇及新发现的词汇)。结果：1.通过对数据库的完善,初步形成了以古代推拿正骨文献为基础的正骨推拿理论框架结构,包括中理法方术四个方面的内容。2.以骨相关理论(骨骼框架、疾病名称、五脏症状、筋骨并重)为例的框架体系。3.建立了正骨手法理论的框架。4.建立了正骨工具理论的框架。5.建立了正骨方剂的框架。结论：1.初步发现古代文献中注重“筋骨并重”的治疗特点。2.通过从基础理论、诊疗方法、辅助方药、操作技法等,采用中医临床诊疗思路的“理”、“法”、“方”、“术”四个方面作为顶层框架进行了研究,以有别于传统文献研究中采用历史源流的纵向研究的方式,通过对古代推拿文献进行了整理,形成了“理法方术”的框架结构。
