加味大承气汤对急性重症胰腺炎患者凝血功能作用的疗效观察

【摘要】：目的观察加味大承气汤联合常规治疗对急性重症胰腺炎患者凝血功能方面的影响。方法 2015年12月-2016年12月期间收治住院的急性重症胰腺炎60例患者,采取随机数字表法分为对照组(30例)和研究组(30例),两组患者均采用常规对症支持治疗,在对照组的基础上,研究组加用加味大承气汤,观察中医证候疗效、生化及凝血指标等,评价加味承气汤对急性重症胰腺炎凝血功能的作用。结果治疗后研究组有效治愈率(96.67%)明显高于对照组(80.00%)(P0.05);治疗后两组APACHEⅡ评分、血清AMY均较治疗前下降,且研究组降低水平大于对照组(P0.05,P0.01)。血清钙浓度逐渐增加,且治疗组的增加幅度大于对照组(P0.05)。两组血凝功能示研究组PT、APTT下降较对照组明显(P0.05),FIB下降不明显。结论加味承气汤对急性重症胰腺炎患者有一定的临床疗效,可改善患者凝血功能,血液高凝状态,故中医药在临床治疗胰腺炎凝血异常中值得推广运用。
[Abstract]:Objective to observe the effect of modified Dachengqi decoction combined with routine therapy on coagulation function in patients with severe acute pancreatitis. Methods 60 patients with severe acute pancreatitis were admitted from December 2015 to December 2016. The study group was divided into control group (n = 30) and study group (n = 30). The patients in both groups were treated with routine symptomatic support. On the basis of the control group, the study group was treated with modified Dachengqi decoction to observe the curative effect of TCM syndromes. To evaluate the effect of Jiawei Chengqi decoction on coagulation function of severe acute pancreatitis, the effective cure rate of the study group was 96.677.Results the effective cure rate of the study group was significantly higher than that of the control group (P 0.05).
