电针不同频率缓解围绝经期烘热症状的临床研究

【摘要】:目的:观察低频电针和高频电针治疗以烘热为主诉的PMS患者的血清内分泌激素(E2、FSH、LH)的影响以及平均24小时烘热积分、MRS量表、MENQOL量表评分指数的变化,以明确不同频率的电针刺激参数缓解围绝经期烘热症状的疗效,评价电针对围绝经期患者烘热症状的治疗效应与频率这一刺激参数的关系以及探讨电针不同频率在缓解围绝经期烘热症状中的疗效和作用机理。方法:本课题在湖北省中医院针灸科招募志愿者,将符合“绝经过渡期标准”并以烘热为主诉的60例围绝经期患者纳入课题研究,按1:1比例随机分为电针高频组(30例)、电针低频组(30例)。两组均针刺关元、子宫、三阴交,所用电针治疗仪为华佗牌SDZ-V型。刺激参数:电针高频组频率为100Hz,电针低频组为2Hz,电流大小以患者能耐受最大为度,每次治疗时间为30分钟,两组都是一周3次,隔日一次,4周为1疗程,持续治疗2个疗程。评价时间点选取治疗前及治疗后,以患者的E2、FSH、LH水平以及平均24小时烘热积分、MRS量表、MENQOL量表评分为观察指标以评价两组的临床疗效。结果:2个疗程治疗完成后,低频组总有效率为96.67%,高频组总有效率为73.33%,疗效差异具有统计学意义(P0.05),电针低频组疗效比高频组显著。电针低频组与高频组的平均24小时烘热积分、MRS量表、MENQ0L量表评分在治疗前后组内自身比较以及治疗后组间比较,差异均有统计学意义(P0.05),电针低频组各项指数下降幅度比高频组各项指数下降更明显,提示低频电针治疗PMS在改善患者烘热症状、临床症状、生存质量方面,疗效比电针高频组好。低频组和高频组治疗后血清E2含量均明显升高,FSH、LH均明显降低(P0.05);治疗后两组血清E2、FSH、LH水平差异组间对照无统计学意义(P0.05),说明两组对PMS患者性激素的影响程度相当。结论:在缓解围绝经期烘热症状方面,低频电针对平均24小时烘热积分、MRS量表、MENQOL量表评分指数改善作用显著,且优于高频电针组,对血清E2、FSH、LH同样具有调节作用。
[Abstract]:Objective: to observe the effect of low frequency electroacupuncture and high frequency electroacupuncture on the serum endocrine hormone E2FSHLH in patients with PMS, and to observe the change of the mean 24 hour dry heat score (MRS) and MENQOL score index. The efficacy of different frequency electroacupuncture stimulation parameters in relieving the symptoms of peri-menopausal fever, To evaluate the relationship between the therapeutic effect and frequency of electroacupuncture on the symptoms of drying heat in peri-menopausal patients, and to explore the therapeutic effect and mechanism of electroacupuncture with different frequencies in relieving the symptoms of peri menopausal fever. Department of Acupuncture and moxibustion, Hubei traditional Chinese Medicine Hospital, According to the proportion of 1: 1, 60 cases of peri-menopausal patients who met the "standard of menopausal transition period" and complained about drying fever were randomly divided into three groups: high frequency electroacupuncture group (30 cases) and electroacupuncture low frequency group (30 cases). The stimulation parameters were as follows: the frequency of high frequency electroacupuncture group was 100 Hz, and that of low frequency electroacupuncture group was 2 Hz. The current intensity was the maximum tolerance of the patient. The time of treatment was 30 minutes each time, and the two groups were 3 times a week. Every other day for 4 weeks as a course of treatment, continuous treatment for 2 courses. Evaluation time points before and after treatment, The clinical efficacy of the two groups was evaluated by the level of FSHLLH and the mean 24 hour dry heat score (Mrs) and MENQOL scale. Results: after two courses of treatment, the clinical efficacy of the two groups was evaluated. The total effective rate of low frequency group was 96.67 and that of high frequency group was 73.33. The difference was statistically significant (P 0.05). The curative effect of low frequency electroacupuncture group was significantly higher than that of high frequency group. Self comparison before and after treatment and comparison between groups after treatment, All the differences were statistically significant (P 0.05). The decrease of each index in low frequency electroacupuncture group was more obvious than that in high frequency group. It was suggested that low frequency electroacupuncture could improve the symptoms of heat drying, clinical symptoms and quality of life in patients with PMS. The effect was better than that in the high frequency electroacupuncture group. The serum E2 level in the low frequency group and the high frequency group were significantly increased after treatment, and the levels of serum E 2 FSH LH in the two groups were significantly lower than those in the control group, indicating that there was no significant difference between the two groups in the level of serum E 2 and E 2 FSH LH, indicating that the two groups had sexual shock in the patients with PMS. Conclusion: in relieving the symptoms of peri-menopausal drying fever, Low frequency electroacupuncture significantly improved the score index of MRS and MENQOL in the mean 24 hours dry heat score and was better than that in the high frequency electroacupuncture group.
