国医大师邓铁涛学术经验传承研究

【摘要】：目的：名老中医学术经验的传承研究一直受当代中医学界重视。2009年,在表彰首届国医大师之后,对国医大师的传承研究再次成为中医药界的研究热点。名老中医是中医学的一种特殊现象,一位名老中医往往能够培养出一批新的名医,并在周围地域相对集中,从而形成“名医圈”现象。当代以名老中医为核心的学术团队成为新型学术流派的雏形逐渐形成。对国医大师学术传承脉络及学术演变进行整理研究亟需开展。目前对国医大师邓铁涛及其学术团队的研究角度多样,但邓铁涛学术传承谱系及其学术经验演变的研究则显不足。本研究对邓铁涛及其学术团队的学术资料进行收集整理,客观呈现其学术传承领域；选择其主要的学术领域作为研究对象,探讨其传承情况及学术演变；选择最具特色的领域对其具体内涵做深入阐述；梳理邓铁涛学术经验传承谱系；探讨其学术传承特点。通过这些研究,存史纪实,彰显邓铁涛及其学术团队的学术成就,为未来传承、发扬邓铁涛教授学术经验提供借鉴与启示。方法：1、文献调研法：包括文献的查找,资料的收集、鉴别、整理、汇编等,形成整个研究工作的基础研究素材。2、理论评述法：综合运用归类分析、归纳演绎、传统阐释、相关对比等方法,以《中医杂志》、《中国中医基础医学杂志》、《中华医史杂志》、《中医文献杂志》关于理论凝练研究方法为行业标杆,按照其行文规范的要求,对邓铁涛教授及其传承人的学术经验进行凝练对比。3、人物访谈法：通过采访相关学者,获得鲜活的历史资料,弥补单纯文献资料的不足。4、数据统计法：采用频数统计、数据挖掘等方法,分析相关方药以发现方药的内在联系。结果：研究分四部分：一为邓铁涛及其学术团队学术资料的收集整理,二为邓铁涛学术经验传承研究,三为邓铁涛诊治重症肌无力学术经验传承研究,四为邓铁涛学术经验传承谱系及传承特点。1、拟定资料纳入、排除标准,共收集到邓铁涛论文248篇,书著42部,第一作者署名非“邓铁涛”、但经广州中医药大学邓铁涛研究所或邓铁涛本人认定是邓铁涛亲撰的书著1部,与邓铁涛相关的其他资料18部；整理邓铁涛学术团队名单共60人及其师从邓铁涛的时间与方式,拟定资料纳入、排除标准,收集其学术资料,较为客观地展现其各自学术领域。根据资料认为,邓铁涛学术团队在五脏相关学说、脾胃学说、中国医学史、岭南医学、中医诊断学、神经肌肉病诊治、心血管病诊治7个领域形成了较为稳定的学术团队并延续至今,故以之为研究对象,探讨邓铁涛在这7个领域学术经验的传承情况及学术演变。2、探讨邓铁涛学术团队在五脏相关学说、脾胃学说、中国医学史、岭南医学、中医诊断学、神经肌肉病诊治、心血管病诊治7个领域的学术传承与演变,关注的重点是学术研究的传承演变而非学术人物的师承关系。对于五脏相关学说,邓铁涛提出了若干原创性的学术观点及研究设想,构建了五脏相关学说的基本理论框架。邓铁涛的学生们在这个框架下进行了多维度的研究,充实了实质性的学术内容,并一定程度上论证了其真实性及学术性。五脏相关学说是一种对中医五行学说的修订,是为了解决当代临床实际问题所提出的一种方案,也是对中医五行学说的现代语言表达。对于脾胃学说,邓铁涛在梳理历代关于脾胃学说的基础上,侧重从临床实践的角度阐述对脾胃学说的理解。这些实践充实了脾胃学说在现代临床各科的适应范围。运用脾胃学说指导消化内科的临床诊疗与科研,是邓铁涛关于脾胃学说学术思想目前的主要传承载体与存在形式。劳绍贤是邓铁涛学术团队中一个独立的分支,是目前邓铁涛学术团队中从事脾胃学说研究的主要代表。对于中国医学史,邓铁涛强调唯物史观与临床实践的导向性,对中医近代史的研究最为重视。临床史观的提出是邓铁涛与其学生在中国医学史研究领域提出的独具特色的治史观念,是其在中国医学史研究领域最具代表性的学术特点。邓铁涛的学生对中医近代史、海外中医药史、中医药博物馆建设等方面的研究不断深入,成果众多。对于岭南医学,目前岭南医学作为全国地域性医学流派研究中的重要一派而颇受重视,得益于邓铁涛及其学生长期的研究作为其基础。邓铁涛的学生始终坚持以传统医史文献方法研究岭南医学,成为目前从医史文献角度研究岭南医学的主要学术团队,其研究成果是运用临床调研、实验研究探讨岭南医学的理论基础。对于中医诊断学,邓铁涛主编的《中医诊断学》奠定了现代中医诊断学的学科范式,邓铁涛的学生在中医诊断学的研究中,一方面对中医诊断学若干受忽视的专题进行了系统研究,另一方面,对邓铁涛多部中医诊断学专著进行修订再版。对于神经肌肉病诊治,从学术理论看,形成了五脏相关学说指导神经肌肉病诊治的鲜明学术特点。从临床实践看,邓铁涛及其学生对重症肌无力、重症肌无力危象的救治的临床影响力辐射全国。在中医诊治神经肌肉病这一领域,邓铁涛及其学生具备形成一个中医学术流派的条件。对于心血管病诊治,从学术理论看,形成了五脏相关学说指导心血管病诊治及重视气虚痰瘀两方面鲜明学术特点。从临床实践看,邓铁涛及其学生在应对不断变化的临床疾病谱的同时,不断加深及拓展五脏相关学说与气虚痰瘀的临床普适性,体现了名老中医学术观点与时俱进的演变历程。在中医诊治心血管内科这一领域,邓铁涛及其学生具备形成一个中医学术流派的条件。基于上述研究认为,邓铁涛学术团队在师徒共同的临床及科研实践中实现了学术的延续与进步。3、整理邓铁涛及其学生诊治重症肌无力经验,展示其学术团队临床现状。邓铁涛对重症肌无力病机的认识为脾胃虚损、五脏相关,当以补脾益气为治疗大法,制强肌健力饮以统治。邓铁涛的学生延续邓铁涛学术风格,在各自长期的临床实践中又形成各自特点。这些积累体现了邓铁涛诊治重症肌无力学术经验在传承之中有新的发展。其团队具备辐射全省乃至全国的临床影响力并在不断增强。收集整理全国名老中医治疗重症肌无力验案,形成44名全国名老中医133则治疗重症肌无力医案汇编,通过对其方药进行频数统计、聚类分析,认为虽然各种理论表述不同,但本质上来看,重症肌无力中医病机属脾肾亏虚、治法以补中益气、用药重用黄芪,这些观点是名老中医对重症肌无力的主流认识。实地调研国内中医界诊治重症肌无力的主要团队并收集其资料,包括上海李庚和、北京黄坤强、浙江裘昌林、河北乞国艳,简介其学术经验与临床现状,认为这些团队在整合医疗资源、中西医结合诊治重症肌无力经验的中医理论总结提升及人才梯队的组建等方面各具优势与特点。通过对比,认为邓铁涛治疗重症肌无力的特色有三：一,运用五脏相关学说指导重症肌无力诊治；二,运用岭南草药进行治疗；三,对重症肌无力危象的中西医结合抢救积累了大量宝贵经验。目前国内中医界诊治重症肌无力的团队的经验与现状也值得邓铁涛及其学术团队借鉴。4、梳理以邓铁涛为核心的中医学学术谱系。共梳理三代,三代依次为邓铁涛授业老师、邓铁涛、邓铁涛的学生,其中第三代又分为合作传承人、代表性传承人、主要传承人三类。学术谱系的建构,对于学派形成、学术研究的延续、学术人才的培养具有至关重要的意义。总结邓铁涛学术团队的传承特点有五：坚信中医,热爱中医；理论临床,兼修并重；执着学术,搭建平台；包容学生,爱护学生；教学相长,后继有人。结论：邓铁涛学术团队在师徒共同的临床及科研实践中实现了学术的延续与进步。邓铁涛在各项研究中往往起着引领作用,开辟多个研究领域,指明研究方向并构建研究的基本框架,而具体的研究内容则多由邓铁涛的学生完成。就研究的深度与广度而言,邓铁涛的学生较邓铁涛均有突破,并使多个研究方向成为当今学界的热点研究问题,也产生良好的临床效应。目前邓铁涛学术团队仍需进一步做好学术的传承,使五脏相关学说、脾胃学说、中国医学史、岭南医学、中医诊断学、神经肌肉病诊治、心血管病诊治等研究领域后继有人,并能在各研究领域继续有所提高。除了在各研究领域继续有所提高之外,应对邓铁涛最独树一帜的学术领域——五脏相关学说及神经肌肉病诊治——进行重点研究。
