基于小分子代谢标志物探讨支气管哮喘寒热证型的辨证特点

【摘要】:研究目的:采用代谢组学方法探索支气管哮喘慢性持续期虚寒证和虚热证患者的小分子代谢物差异,研究揭示哮喘临床证候表型与小分子代谢物之间的相关性,寻找哮喘证型的潜在生物标志物,为中医辨证论治提供有效的靶点。研究方法:选取北京地区汉族哮喘慢性持续期患者20例(虚寒证与虚热证各10例),身体健康的志愿者10例,取得受试者知情同意后,收集空腹外周抗凝血2ml,抗凝静置,离心取血清,-80℃冰箱保存,进行气相色谱-质谱联用(GC-MS)分析,采用多维分析(0)PLS-DA和单维分析(t检验)相结合的办法,来筛选组间差异代谢物(VIP1,p0.05)。通过MBRole与KEGG进行差异代谢物通路富集分析。研究结果:慢性持续期哮喘患者与健康对照组相比,共发现40种差异代谢物;虚寒证组与虚热证组相比,共发现18种差异代谢物;既是哮喘相关小分子代谢物,又可以区分虚寒证和虚热证两种不同证型的小分子代谢物有4种,分别为:二十四碳烷(tetracosane)、4-氨基丁酸(4-aminobutyricacid)、苯甲酸甲酸(Benzoylformicacid)、异肌醇(Allo-inositol)。二十四碳烷主要作为色谱分析标样及固定液;苯甲酰甲酸作为原料,可合成多种药剂,比如胃溃疡用药胃长宁、抗胆碱药阿托品、抗炎药布洛芬等;异肌醇作为化学试剂;4-氨基丁酸主要功能在中枢神经系统、脑部及生殖系统,多种外周组织中也存在,与哮喘的诊治相关。经MBRole与KEGG分析哮喘虚寒证组和虚热证组差异代谢物,发现β-丙氨酸代谢通路受到扰动。结论:1、支气管哮喘慢性持续期患者与健康对照组相比小分子代谢物有显著差异。2、哮喘虚寒证与虚热证患者小分子代谢物有显著差异。3、4-氨基丁酸(4-aminobutyric acid)可能是潜在鉴别支气管哮喘慢性持续期虚寒证和虚热证型的生物标记物。4、哮喘虚寒证组和虚热证组β-丙氨酸代谢通路受到扰动。
[Abstract]:Objective: to explore the differences of small molecule metabolites in asthmatic patients with chronic asthmatic deficiency cold syndrome and deficiency heat syndrome by means of metabonomics, and to reveal the correlation between the phenotype of asthma clinical syndrome and small molecule metabolites. To search for potential biomarkers of asthma syndrome type and to provide effective targets for TCM syndrome differentiation and treatment. Methods: twenty patients with chronic duration of asthma (10 cases of deficiency cold syndrome and 10 cases of deficiency heat syndrome) and 10 healthy volunteers were selected from the Han nationality in Beijing area.
