周文波主任应用扶阳法治疗恶性肿瘤（肾阳亏虚型）临床经验

【摘要】：目的:回顾、总结恶性肿瘤的中西医学病因病机、治疗原则、治疗方法,总结周文波主任治疗恶性肿瘤(肾阳亏虚型)的学术思想及经验,探讨其辨证思路及用药规律,为继承资深老中医治疗恶性肿瘤的宝贵经验,提供第一手临床资料,为临床治疗恶性肿瘤提供新思路。临床资料与方法:以2015年6月——2016年1月期间在辽宁省中医研究院肿瘤科门诊求治的67名(肾阳亏虚型)患者为临床观察对象,以周文波主任自拟的扶阳一号方为基本方治疗,7天为一个疗程,连续服用4个疗程,并进行心理疏导,同时嘱患者日一次自行艾灸关元、神阙、命门穴位,以皮肤潮红为度,大约20分钟。每个疗程后记录并统计患者生存质量评分、KPS评分。治疗结束一个月后随访。4个疗程后,将数据进行统一整理,对患者生存质量评分、KPS评分进行评估,运用spss17.0统计软件进行统计学分析,客观地评价治疗后的临床疗效。结果:统计结果表明:经过四个疗程的治疗后,67例患者(肾阳亏虚型)的KPS及QOL评分均较治疗前升高,其中治疗后第二周、第三周、第四周的KPS评分分别为56.36±7.48、58.68±7.86、61.43±8.29,与治疗前比较有明显差异(P0.05),治疗结束一个月后随访,KPS评分为69.47±9.13,与治疗前比较有显著性差异(P0.01)。治疗后第二周、第三周、第四周的QOL评分分别为28.36±3.71、30.13±4.09、32.58±4.33,与治疗前比较有明显差异(P0.05),经过四个疗程的治疗结束一个月后,再次随访,所记录的QOL评分为39.09±5.38,与治疗前比较有显著性差异(P0.01)。结论:肾阳亏虚型肿瘤多是在阳气耗散枯竭、阴阳交感缺失、阳气不能制约阴形的前提下导致的“阴阳离绝之势”。扶阳法具有补肾益阳、镇静安神等作用,通过扶助阳气,使阳气在卫外过程中,通过阴阳交感、产生精微物质,藏于体内,发挥治疗和防御恶性肿瘤的作用。药物、艾灸治疗的同时,临床还注重心理疏导,缓解患者恐惧、焦虑、抑郁的情绪。使患者阳气渐复,体质改善,对于提高患者生存质量,具有理想的临床疗效。
[Abstract]:Objective: to review and summarize the etiology, pathogenesis, treatment principles and treatment methods of malignant tumors in Chinese and western medicine, summarize the academic thoughts and experiences of Director Zhou Wenbo in the treatment of malignant tumors (deficiency of kidney yang), and discuss its dialectical thinking and drug use law. In order to inherit the valuable experience of senior traditional Chinese medicine in the treatment of malignant tumors, to provide first-hand clinical data, and to provide new ideas for clinical treatment of malignant tumors. Clinical data and methods: from June 2015 to January 2016, 67 patients (deficiency of Kidney Yang) seeking treatment in oncology department of Liaoning Academy of traditional Chinese Medicine were selected as clinical observation objects. Taking the Fuyang No. 1 prescription prepared by Director Zhou Wenbo as the basic prescription for 7 days as a course of treatment, taking four consecutive courses of treatment, and conducting psychological counseling, the patient was told to moxify Guanyuan, Shenque, Mingmen acupoints once a day, with the degree of redness to the skin. In the fourth week, the KPS scores were 56.36 卤7.48 卤58.68 卤7.86 卤61.43 卤8.29, which were significantly different from those before treatment (P0.05), and the follow-up scores of one month after treatment were 69.47 卤9.13, which were significantly different from those before treatment (P0.01). Conclusion: most tumors of deficiency of kidney-yang are caused by the depletion of Yang qi, the deficiency of synaesthesia of yin and yang, and the fact that yang qi can not restrict the Yin shape. Fuyang method has the functions of tonifying the kidney and supplementing yang, calming and calming the mind, and so on. It helps Yang Qi to produce subtle substances in the process of protecting and protecting outside the health, and produces subtle substances, which are hidden in the body and play a role in treating and defending malignant tumors.
