养阴祛风法治疗咳嗽变异性哮喘的临床研究

【摘要】：目的探讨养阴祛风法治疗咳嗽变异性哮喘的临床疗效和安全性。方法入选64例咳嗽变异性哮喘病例,随机将入选的患者分为治疗组和对照组。治疗组(32例)采用养阴祛风方治疗,对照组(32例)采用吸入信必可(布地奈德福莫特罗粉吸入剂),每次1吸,每日2次,疗程均为4周。随机抽样分组,制定观察表,分别于治疗前、治疗第1周、治疗第2周及治疗第4周后记录患者症状积分；治疗前及治疗4周后分别记录肺功能及外周血嗜酸细胞计数。并观察治疗组是否与对照组存在统计学差异。结果治疗4周后,两组治疗前后比较,在症状积分以及肺功能、外周血嗜酸细胞计数方面均有非常显著统计学意义(P0.01)；组间比较,两组患者显控率有显著统计学差异(P0.05),总有效率无统计学意义(P0.05),但在肺功能改善及外周血嗜酸细胞计数均有显著统计学意义(P0.05)。治疗组未见明显不良反应,对照组有2例轻微不良反应。结论养阴祛风方对咳嗽变异性哮喘的临床疗效肯定,尤其在肺功能及外周血嗜酸细胞计数方面改善明显,安全性高。意义导师朱启勇教授总结40余年临床经验,发现风邪犯肺、肺阴亏虚是咳嗽变异性哮喘的主要病机,因此提出的“养阴祛风法”是治疗咳嗽变异性哮喘的有效治法,养阴祛风法能全面改善患者咳嗽及伴随症状,提高患者生活质量,具有病情不容易反复且副作用少的特点,值得临床推广应用。
[Abstract]:Objective to explore the clinical efficacy and safety of tonifying yin and removing wind in treatment of cough variant asthma. Methods 64 patients with cough variant asthma were randomly divided into treatment group and control group. The treatment group (32 cases) was treated with Yangyin Qufeng recipe, while the control group (32 cases) was treated by inhaling budesonide formoterol powder for 1 dose twice a day for 4 weeks. The symptom scores were recorded before treatment, 1 week, 2 weeks and 4 weeks after treatment, and the lung function and eosinophil count in peripheral blood were recorded before and 4 weeks after treatment, respectively. And observe whether there is statistical difference between the treatment group and the control group. Results after 4 weeks of treatment, there were significant differences in symptom score, lung function and peripheral blood eosinophil count between the two groups before and after treatment (P0.01). There was significant difference between the two groups (P0.05), but the total effective rate was not statistically significant (P0.05), but there was significant difference in lung function improvement and peripheral blood eosinophil count (P0.05). There were no obvious adverse reactions in the treatment group, while in the control group there were 2 cases of mild adverse reactions. Conclusion Yangyin Qufeng recipe is effective and safe in treating cough variant asthma, especially in lung function and eosinophil count in peripheral blood. Significance mentor Professor Zhu Qiyong summed up more than 40 years clinical experience, found that wind evil invades lung, lung yin deficiency is the main pathogenesis of cough variant asthma, so "nourishing yin to remove wind" is an effective treatment for cough variant asthma. The method of nourishing yin and removing wind can improve the cough and accompanying symptoms and improve the quality of life of the patients. It has the characteristics of less side effects and less recurrence. It is worth popularizing and applying in clinical practice.
