循经弹拨法治疗膝关节骨性关节炎的临床研究

【摘要】：目的:本课题旨在通过对循经弹拨法和口服盐酸氨基葡萄糖胶囊两种方法治疗膝关节骨性关节炎的疗效进行对比分析,进而研究循经弹拨法治疗膝关节骨性关节炎的临床疗效及作用机理,为临床治疗提供科学理论依据。方法:课题共纳入膝关节骨性关节炎患者72例,按照随机对照的原则将患者分为治疗组(循经弹拨组)和对照组(口服盐酸胺基葡萄糖胶囊组),在治疗过程中共有7例患者由于不同原因脱落,最终治疗组实际共治疗患者32例,对照组实际治疗患者33例;治疗组每日治疗一次,15min/次,六天为一疗程,疗程间休一天,共治疗四个疗程;对照组口服盐酸葡萄糖氨基胶囊进行治疗,每次2粒,一日三次,七天为一疗程,共治疗四个疗程。每例患者分别于治疗前、治疗14天、28天后进行膝关节活动角度测量、Lysholm膝关节功能评分测定,并于治疗前和治疗后对膝关节局部IL-1、TNF-α含量进行测定。记录测量、理化检查结果并通过统计学分析数据,比较两组患者膝关节功能症状的改善以及预后情况。结果:经统计学软件检验,治疗组与对照组患者的年龄、性别以及症候分布方面均无显著性差异,具有可比性。根据疗效判定标准判定,治疗组总有效率为87.5%,控显率为71.9%;对照组总有效率为72.7%,控显率为48.5%。经过统计学处理得出两组疗效比较差异有统计学意义(P0.05)。结论:循经弹拨法治疗膝关节骨性关节炎是一种安全、有效的治疗方法,其疗效与口服西药盐酸胺基葡萄糖胶囊比较相对较好,并且在改善膝关节活动角度等方面明显优于对照组,具有明显优势。循经弹拨推拿法治疗膝关节骨性关节炎操作方便、易于学习且患者容易接受,值得作进一步临床研究继而在临床推广。
[Abstract]:Objective: the purpose of this study was to compare and analyze the curative effect of Xuan Meridian bouncing method and oral glucosamine Hydrochloride capsule in the treatment of knee osteoarthritis, and to study the clinical efficacy and mechanism of the treatment of knee osteoarthritis, so as to provide scientific theoretical basis for clinical treatment. Methods: a total of 72 patients with knee osteoarthritis were divided into treatment group (group B) and control group (group B) according to the principle of random control. A total of 7 patients fell off due to different reasons in the course of treatment. 32 patients in the final treatment group and 33 patients in the control group were actually treated. The treatment group was treated with 15min/ once a day, six days as a course of treatment, one day between the courses of treatment, a total of four courses of treatment, while the control group was treated with glucosamine hydrochloric acid capsule, 2 tablets at a time, three times a day, seven days as a course of treatment, a total of four courses of treatment. The knee joint movement angle and Lysholm knee function score were measured before treatment, 14 days and 28 days after treatment, and the local IL-1,TNF- 伪 content of knee joint was measured before and after treatment. The results of measurement and physical and chemical examination were recorded and statistically analyzed to compare the improvement and prognosis of knee joint function between the two groups. Results: there was no significant difference in age, sex and syndrome distribution between the treatment group and the control group by statistical software, and there was no significant difference between the treatment group and the control group in age, sex and syndrome distribution. According to the criteria of curative effect, the total effective rate was 87.5% and the controlled display rate was 71.9% in the treatment group and 72.7% and 48.5% in the control group. After statistical treatment, it was found that there was significant difference between the two groups (P 0.05). Conclusion: the method of bouncing along meridians is a safe and effective method for the treatment of knee osteoarthritis. Its curative effect is relatively better than that of oral administration of aminoglucose Hydrochloride capsule, and it is obviously superior to the control group in improving the movement angle of knee joint. The treatment of knee osteoarthritis with elastic massage is convenient, easy to learn and easy for patients to accept, which is worthy of further clinical research and clinical promotion.
