【摘要】：我国于1999年开始实行执业医师资格考试制度,执业医师资格考试制度是《中华人民共和国执业医师法》规定的,由卫生行政主管部门负责组织实施的医疗卫生行业准入性考试制度,医师资格考试是在我国社会主义市场经济不断发展成熟的大背景下最终确立的,执业资格考试是国家实行行业准入制度的前提,也是发展成熟、规范的社会主义市场经济的必然选择。执业医师资格制度的建立,是我国政府对医疗卫生行业实行专业技术人员准入控制的重要举措,是相对科学公平的评价制度。根据《中华人民共和国执业医师法》,《中华人民共和国执业医师法》明确规定医学院校毕业生必须通过执业医师资格考试方可取得行医资格。执业医师考试制度对提高我国医师队伍的整体素质,推动医学院校教育质量的提升以及为执业医师的国际接轨与交流进行了大胆的尝试,为我国医疗卫生事业的发展进步作出了重要的贡献,在中国医师发展史上具有不可磨灭的影响。但由于我国医师资格考试制度起步较晚,考试结构和考试管理体系还不够健全,多年来,各级考试管理机构不断完善医师资格准入标准,积极探索适合中国国情的考试模式,发挥对医学教育的引导作用,使医师资格考试的管理专业化、规范化和科学化水平不断得到提高,但仍然还面临着很多的问题,不论从考试形式、考试范围和考试内容,还是从考试的组织管理等方面,与发达国家较为成熟的医师执业考试相比,都存在较大差距,亟需规范、完善和发展。笔者采用文献研究、访谈的方法,综合分析我国目前执业医师资格考试的现状,特别是对存在的问题提出了解决对策,旨在为执业医师资格考试工作者提供有益的借鉴。此文章共五个章节。第一章为绪论。介绍我国执业医师资格考试的背景、研究对象及对国内外的文献的回顾。笔者对国内外文献进行回顾,通过我国执业医师资格考试与美国等实行执业医师资格考试制度较早的国家进行对比,找出我国在考试内容、组织管理等方面与其他国家的差距;对国内文献的回顾主要集中在目前我国医师资格考试管理存在考试制度和模式单一、考试管理方面的法制建设相对薄弱、在考务管理人员的培训力度方面有待进一步加强及考试诚信体系缺失等方面的问题进行回顾,深入了解目前我国执业医师资格考试的研究动态,同时结合公共治理理论、新公共管理理论以及第三部门等相关的管理理论内容,较为详细相关管理理论与执业医师资格考试管理的关联性进行了阐述,为后续文章的写作提供了理论依据,打下了坚实的基础。第二章,介绍执业医师资格考试管理的现状。先后对我国执业医师资格考试制度的发展进程进行了梳理;总结出在我国现行的执业医师资格考试管理中,实践技能考试管理与笔试考试管理还不够均衡;考试组织管理工作体系有待于进一步规范、考务管理人员素质有待进一步提高等状况。第三章,介绍我国执业医师资格考试管理工作存在的问题。本章主要介绍我国执业医师资格考试管理中存在的报名资格规定专业与院校专业设置匹配度不高、实践技能考试管理规范化程度偏低、考试通过率不能满足医疗卫生事业发展和人民就医需求、国际化认可度低,考试违规违纪现象频发等方面的问题。第四章,提出优化医师资格考试管理的举措。一是政府充分发挥社会管理职能,完善医师资格考试管理的法制和制度建设,加强对学校的引导和管理,督促考试管理部门建立标准化的评分标准,建设标准化的考试基地,不断探索医师资格考试的考评方式和考试模式;二是政府要充分发挥社会服务的相关职能。利用现代技术,提高模拟真实性,提高考生的受教育水平,从严把控资格审核;三是多部门联动,完善执业医师资格考试管理工作。政府加强资金和政策支持,充分发挥行业组织的作用,同时探索执业医师资格分级管理制度;四是对考试内容和题目设计进行完善和改革。增加人文社会科学知识、预防医学知识和数据应用能力的考察,借鉴实行医师考试度较早的国家的先进经验,加快我国医师从业人员与国际接轨的进程,提高我国医师资格考试的国际认可度;五是多方面入手,改善考试环境,加强考试诚信体系建设,推行无纸化考试,不断改善考试环境,有效预防考试违规违纪现象的发生。第五章,总结。本章主要对整个文章的研究内容和组织架构以及研究结论进行回顾,并指出研究的不足之处。
[Abstract]:In 1999, China began to practise the qualification examination system of practicing physicians. The qualification examination system of practicing physicians is stipulated in the law of People's Republic of China practitioner, and the medical and health industry admittance examination system is organized by the Department of health administration, and the examination of physician qualification is continuously developed in the socialist market economy of our country. Under the big background, the qualification examination is the prerequisite for the country to implement the industry admittance system. It is also the inevitable choice for the mature and standardized socialist market economy. The establishment of the licensed practitioner qualification system is an important measure for the government to implement the access control of the professional and technical personnel in the medical and health industry. It is a relative section. According to the law of People's Republic of China practitioner, the law of People's Republic of China practitioner clearly stipulates that the graduates of medical colleges and universities must obtain the qualification of practicing medicine through the qualification examination of medical practitioners. The examination system of practicing physicians will improve the overall quality of the medical team in our country and promote the quality of education in medical colleges and universities. It has made an important contribution to the development and progress of Chinese medical and health services, and has an indelible influence on the history of Chinese physicians. However, because of the late start of the medical qualification examination system in China, the structure of examination and the system of examination management are not healthy enough. All over the years, examination management institutions at all levels have continuously perfected the standards for admission to doctors, actively explored the mode of examination suitable for China's national conditions, played a guiding role in medical education, made the management of the examination of physicians professionally specialized, standardized and scientifically improved, but still faced many problems, regardless of the form of examination. In terms of the scope and content of the examination or the organization and management of the examination, there is a big gap between the mature physician practice examination in the developed countries, and it is urgently needed to be standardized, perfected and developed. The author adopts the literature research, the interview method, and analyzes the present situation of the qualification examination of the medical practitioner in our country, especially the existence of the present situation. The problem puts forward the solutions to provide useful reference for the practitioner qualification examination workers. This article is the introduction of five chapters. The first chapter is the introduction. It introduces the background of the qualification examination of Chinese practitioners, the research object and the review of the literature at home and abroad. Compared with the countries of the United States and other countries which have practiced the qualification examination system earlier, we can find out the gap between China's examination content, organization and management and other countries. The review of domestic literature mainly focuses on the present examination system and mode of the examination management of physicians in our country and the legal construction of examination management. A review is made on the problems of weakness, further strengthening of the training of the administrative staff and the lack of integrity system in examination, and a thorough understanding of the current research trends in the qualification examination of the licensed physicians in our country, and in combination with the theory of public governance, the theory of new public management and the management theory related to the third departments. This paper expounds the relevance between the detailed related management theory and the management of the qualification examination of the licensed physicians, provides a theoretical basis for the writing of the following articles, and lays a solid foundation. The second chapter introduces the status of the management of the qualification examination of the licensed physicians. In the current Chinese medical qualification examination management, the practice skill examination management and the written examination management are not well balanced; the examination organization and management work system needs further standardization, the quality of the examination management personnel needs to be further improved. The third chapter introduces the problems existing in the management of the qualification examination of the licensed physicians in our country. This chapter mainly introduces the qualification of qualification examination in the qualification examination of medical practitioners in China, which stipulates that the matching degree between the professional and the colleges and universities is not high, the standardization of the test management of practical skills is low, the passing rate of the examination can not meet the development of medical and health services and the needs of the people for medical treatment, the international recognition is low, and the violation of discipline is frequent. In the fourth chapter, we put forward the measures to optimize the management of doctor qualification examination. First, the government should give full play to the social management function, improve the legal system and system construction of the management of the doctor qualification examination, strengthen the guidance and management of the school, supervise the examination management department to establish the standardized scoring standard, and build a standardized test base, and continue to build a standardized examination base. To explore the evaluation and examination mode of doctor qualification examination, the two is that the government should give full play to the relevant functions of social service. Using modern technology to improve the authenticity of the simulation, improve the education level of the examinee, strictly control the qualification examination; three is the linkage of many departments, perfect the management of the qualification examination of the practitioner. The government strengthens the fund and the policy. Support, give full play to the role of the industry organization, and explore the classification management system for the qualification of medical practitioners; four is to improve and reform the design of examination content and subject, to increase the knowledge of Humanities and Social Sciences, to prevent medical knowledge and the application of data, to learn from the advanced experience of the country which is earlier in the medical examination and to accelerate our country. Medical practitioners and international integration process to improve the international accreditation of Chinese physician qualification examination; five is to start with many aspects, improve the examination environment, strengthen the construction of examination and credit system, carry out paperless examination, constantly improve the examination environment, effectively prevent the occurrence of violation of rules and regulations. This chapter summarizes the whole article. The research contents, organizational structure and research conclusions of the chapter are reviewed, and deficiencies of the research are pointed out.
