超声评估女子水球运动员肩峰下滑囊损伤的实践研究

【摘要】:目的:分析超声图像特征,比较肩峰下滑囊厚度。总结水球运动员肩部损伤规律,探讨超声诊断技术在评估运动员肩部健康状况以及排查、预防、治疗肩部伤病等方面的实用价值。方法:募集成都体育学院附属竞技体校女子水球队运动员。进行肩部超声检查,测量肩峰下滑囊厚度。依据检查结果将各单侧肩关节划分为有症状组与无症状组。募集非运动员的健康普通人优势侧肩50例作对照组。分析超声图像特征,采用T检验比较肩峰下滑囊厚度差异。结果:24名运动员纳入研究,48例样本肩关节中,30例未查出异常,18例查出损伤症状,诊断肩关节局部损伤,水球运动员肩关节损伤率为37.5%。18例损伤肩中查出各型伤病24例,含肩峰下滑囊炎症17例;肱二头肌长头腱鞘炎症5例;冈上肌腱炎症1例;三角肌撕裂伤1例;另查出肩关节疑似损伤5例。测得运动员左侧肩峰下滑囊平均厚度2.58±0.58mm,右侧肩峰下滑囊平均厚度2.40±0.44mm。差异无统计学意义(P=0.178)。运动员有症状组肩峰下滑囊平均厚度2.93±0.42mm,运动员无症状组肩峰下滑囊平均厚度2.21±0.36mm,差异具有统计学意义(P0.01)。运动员有症状组肩峰下滑囊厚度明显大于无症状组。对照组肩峰下滑囊平均厚度1.94±0.28mm,运动员有症状组肩峰下滑囊厚度均数与对照组比较,差异具有统计学意义(P0.01);运动员无症状组肩峰下滑囊厚度均数与对照组比较,差异具有统计学意义(P0.01)。与健康普通人相比,水球运动员肩峰下滑囊厚度明显偏大。结论:1、成都体育学院附属竞技体校女子水球队运动员48例肩关节中,18例患肩关节局部损伤,患病率37.5%,建议尽快采取防治措施。2、水球运动员肩关节超声图像具有群体特征。表现为肩峰下滑囊厚度较健康普通人偏大,滑囊损伤多表现为厚度增加和积液;肩袖肌腱状况总体良好;肱二头肌长头腱状况总体良好;但存在疑似损伤病例。3、超声诊断技术具有独特优势。能够深入运动队驻地完成检查任务,体现了超声诊断准确可靠、方便快捷的特点。通过超声检查诊断了既有损伤,排查和鉴别了疑似损伤。超声诊断技术在运动员损伤排查方面,以及指导治疗和预防损伤方面具有实用价值。建议推广使用超声诊断技术排查运动损伤的方法。
[Abstract]:Objective: to analyze the characteristics of ultrasound images and compare the thickness of subacromial bursa. This paper summarizes the law of shoulder injury of water polo players, and probes into the practical value of ultrasonic diagnosis technology in evaluating the shoulder health status of water polo athletes, as well as in the investigation, prevention and treatment of shoulder injuries. Methods: the female water polo team athletes of Chengdu Institute of physical Education were recruited. The thickness of subacromial bursa was measured by shoulder ultrasound. According to the results of examination, each unilateral shoulder joint was divided into symptomatic group and asymptomatic group. 50 healthy ordinary people who recruited non-athletes had the advantage of lateral shoulder as the control group. The ultrasonic image features were analyzed and T test was used to compare the thickness difference of subacromial bursa. Results: 24 athletes were included in the study. Of the 48 samples of shoulder joint, 30 cases were not abnormal, 18 cases were found injury symptoms, and the local injury of shoulder joint was diagnosed. The injury rate of shoulder joint of water polo athletes was 37.5%. Among 18 cases of shoulder injury, 24 cases were found to be injured and 17 cases were included subacromial bursa inflammation. There were 5 cases of long head tendon sheath inflammation of biceps brachii muscle, 1 case of supraspinous tendon inflammation, 1 case of deltoid muscle laceration, and 5 cases of suspected shoulder joint injury. The average thickness of the left subacromial bursa was 2.58 卤0.58mm, and that of the right subacromial bursa was 2.40 卤0.44mm. There was no significant difference (P 鈮,
