山东省新型农村合作医疗制度运行绩效评估

【摘要】：从“三农问题”的角度出发,2002年党中央、国务院将农民健康水平的提高与农村卫生状况的改善作为统筹城乡区域经济社会发展的重大举措,通过了建立全民覆盖的新型农村合作医疗制度的战略目标。山东省作为农业大省,于2003年率先启动了新型农村合作医疗制度试点工作,取得了一定的成效。但是在试点工作过程中,难免暴露出部分问题,因此,如何对新农合制度的运行绩效进行合理有效地评价可以推动其持续稳健发展。本文回顾和梳理了新型农村合作医疗制度相关的基本理论,并以此为基础分别从参合情况、受益情况、筹资情况等方面分析了2011年山东省新农合制度的运行状况及暴露出的问题。同时采用因子分析法对山东省新农合制度的运行进行了绩效评估。在此过程中创新性地以逻辑模型为基础并引入绩效评价“4E”准则,从投入、过程、产出和影响四个方面来设计完整的指标体系。最终选取了参合率、住院补偿比、人均筹资等13个指标,以山东省17地市为实证对象对其新型农村合作医疗制度的运行绩效进行了定量评估。实证研究发现：新农合的运行效率与地域分布有较大关系。综合来看,新农合的东西差异与南北差异明显,总体呈现出“东高西低,北高南低”的特点和地域差异,这与公平性相悖,我省应注重公平,提高新农合制度的总体绩效。本文科学客观地评价新型农村合作医疗制度的运行绩效,对于相关部门制定决策和绩效考评有一定的理论意义。本文深入分析山东省新农合制度运行的状况和存在的问题,有助于完善山东省的基本社会保障、推动社会主义新农村建设和山东省新农合的持续健康发展。
[Abstract]:From the point of view of the "three rural issues", in 2002, the CPC Central Committee and the State Council took the improvement of farmers' health level and the improvement of rural health status as a major measure to co-ordinate the regional economic and social development of urban and rural areas. The strategic goal of establishing a new rural cooperative medical system with universal coverage was adopted. Shandong Province, as a large agricultural province, took the lead in launching the pilot work of the new rural cooperative medical system in 2003. However, some problems are inevitably exposed in the process of pilot work. Therefore, how to evaluate the performance of NCMS reasonably and effectively can promote its sustainable and steady development. This paper reviews and combs the basic theories related to the new rural cooperative medical system, and on this basis, respectively from the situation of participation, benefit, In 2011, the paper analyzes the operation of the new agricultural cooperation system in Shandong province and the problems it has exposed. At the same time, it evaluates the performance of the new agricultural cooperation system in Shandong province by using factor analysis method. Based on the logical model and introducing the "4e" criterion of performance evaluation, A complete index system is designed from four aspects: input, process, output and impact. Finally, 13 indicators, such as participation rate, hospitalization compensation ratio and per capita financing, are selected. Taking 17 cities of Shandong Province as the empirical object, the paper quantitatively evaluates the performance of the new rural cooperative medical system. The empirical study shows that the operational efficiency of the new rural cooperative medical system is closely related to the regional distribution. The difference between east and west of NCMS is obvious with the difference between north and south, which shows the characteristics and regional difference of "east high and west low, north high and south low", which is contrary to the fairness, so we should pay attention to fairness and improve the overall performance of the new agricultural cooperation system. This liberal arts science objectively evaluates the performance of the new rural cooperative medical system, which has certain theoretical significance for the relevant departments to make decisions and performance evaluation. This paper deeply analyzes the operating status and existing problems of the new rural cooperative medical system in Shandong Province. It is helpful to perfect the basic social security of Shandong province and promote the construction of new socialist countryside and the sustainable and healthy development of Shandong new rural cooperation.
