我国城乡居民大病保险筹资模式研究

【摘要】:随着200年新医改在全国推广以来,到目前为止,社会基本医疗保险体系基本完成。也就是包括城镇职工医疗保险、城镇居民医疗保险、新型农村合作医疗保险在内的基本医保体系初步建立。根据2012年中国统计年鉴数据显示,截至到12月底,参加社会基本医疗保险的人数已经超过13亿人,基本实现全覆盖。而重大疾病所导致的大额的医疗费用依然是对每一个家庭的潜在威胁,因病致贫和因病返贫依然是每个家庭挥之不去的梦魇。 为了解决这一突出问题,2012年8月,国务院出台了《关于开展城乡居民大病保险工作的指导意见》(以下简称《指导意见》),这项文件的出台为各地展开城乡居民大病保障工作提供了政策性的依据。《指导意见》指出,在确保基本医保能够稳定运转的同时,能够将城乡居民基本医保的基金结余的一定比例作为大病保险的筹资来源。 想要大病保险长期的持续运行下去,,从根本上解决因病致贫,因病返贫的困境,首先也是最主要的就是要解决大病保险的筹资问题。本文首先从理论上将我国城乡居民大病保险筹资模式界定为筹资来源、筹资标准以及统筹层次和范围。然后通过对当前试点地区大病保险的四种典型模式分别研究其筹资模式的类型和可行性。最后一部分给出大病保险的政策性意见。本文结论主要有完善大病保险筹资渠道;建立大病保险基金独立运作的平台;合理确定不同地区大病保险的筹资标准;适时的扩大大病保险的统筹层次和范围。
[Abstract]:Since the new medical reform was popularized in the whole country in 200 years, the basic social medical insurance system has been basically completed. That is, the basic medical insurance system, including urban worker medical insurance, urban resident medical insurance and new rural cooperative medical insurance, was initially established. By the end of December, the number of people participating in basic social medical insurance had exceeded 1.3 billion, according to data from the China Statistical Yearbook of 2012, basically achieving full coverage. However, the high cost of medical care caused by major diseases remains a potential threat to every family, and poverty caused by illness and return to poverty by illness remain a constant nightmare for every family. In order to address this outstanding problem, in August 2012, The State Council has issued the guiding opinion on the Development of serious illness Insurance for Urban and Rural residents (hereinafter referred to as "the guiding opinion"), which provides a policy basis for all localities to carry out the work of ensuring serious illness among urban and rural residents. While ensuring the stable operation of the basic medical insurance, we can take the proportion of the fund balance of the basic medical insurance of urban and rural residents as the source of financing for the serious illness insurance. Want the serious illness insurance to run continuously for a long time, fundamentally solve the predicament of poor caused by illness and return to poverty because of illness, the first and most important is to solve the financing problem of the serious illness insurance. Firstly, this paper defines the financing mode of urban and rural residents' serious illness insurance in theory as the source of financing, the standard of financing, the level and scope of overall planning. Then the paper studies the types and feasibility of the financing model of the four typical models of the major illness insurance in the current pilot area. The last part gives the policy advice of serious illness insurance. The main conclusions of this paper are to improve the financing channels of the serious illness insurance; to establish a platform for the independent operation of the insurance fund for serious illness; to rationally determine the financing standards of the serious illness insurance in different regions; to expand the overall planning level and scope of the serious illness insurance in good time.
