黑龙江省新型农村合作医疗制度运行的问题及对策研究

【摘要】：我国传统农村合作医疗制度在维护农民的健康、满足农民的医疗基本需求和减轻农民的医疗负担中发挥了重要的作用。但随着我国经济突飞猛进的发展，城乡二元结构矛盾的日益突出，城乡差距进一步拉大，因此导致的农民“因病致贫，因病返贫”的现象也越来越严重。面对这种日益严重的现象，我国在2003年，推出了由政府组织、引导、支持，农民自愿参加，个人、集体和政府多方筹资，以大病统筹为主的农民医疗互助共济制度。也就是新型农村合作医疗制度。这与我国传统农村合作医疗制度具有本质上的不同。新制度的推出对农民抵御重大疾病，改善农民生活质量，促进农村社会经济发展具有重要意义。其研究的目的是通过对黑龙江省新型农村合作医疗制度运行现状进行研究，结合国内外农村医疗保障制度的成功经验，为黑龙江省相关部门提供可行性意见和对策，从而进一步完善和加强黑龙江省新型农村合作医疗制度的实施和发展；在研究过程中也有创新之处。一是在分析的角度上创新，分析黑龙江省新型农村合作医疗制度运行中存在的问题原因时，从新型农村合作医疗制度本身的缺陷和阻碍黑龙江省新型农村合作医疗制度运行的障碍这两个角度入手分析。二是对完善黑龙江新型农村合作医疗制度的对策提出了几点创新。首先，对国内外研究现状进行系统的评述，提出论文基本框架和创新点；其次，明晰了相关的概念和理论依据；然后，通过对黑龙江省新型农村合作医疗制度运行情况进行系统的分析整理，指出了黑龙江省新型农村合作医疗制度存在的问题，如资金筹集难、实际补偿低、监管能力低等。并从新型农村合作医疗制度本身的缺陷和黑龙江省新型农村合作制度的运行的障碍这两个角度入手分析其成因；再次，通过借鉴对国外农村医疗保障制度和国内某些地区的新型农村合作医疗制度运行的先进经验，得到一些有利于黑龙江新型农村合作医疗制度发展的启示；最后，是提出可行性对策。结合黑龙江省的实际情况，，在筹资机制，监管机制，补偿方案等方面提出相关对策来进一步完善新型农村合作医疗制度在黑龙江省的运行。
[Abstract]:The traditional rural cooperative medical system plays an important role in maintaining farmers' health, satisfying their basic medical needs and lightening their medical burden. However, with the rapid development of economy in China, the contradiction between urban and rural dual structure is becoming more and more prominent, and the gap between urban and rural areas is further widened. Therefore, the phenomenon of peasants "getting poor because of illness and returning to poverty due to illness" is becoming more and more serious. In the face of this increasingly serious phenomenon, in 2003, our country introduced the farmers' medical mutual aid system, which is organized, guided, supported, voluntarily participated by farmers, raised funds from individuals, collectives and governments, and focused on the coordination of serious illness. That is, the new rural cooperative medical system. This with our country traditional rural cooperative medical system has the essential difference. The introduction of the new system is of great significance for farmers to resist major diseases, improve their quality of life and promote the development of rural society and economy. The purpose of this study is to provide feasible suggestions and countermeasures for the relevant departments of Heilongjiang Province through the study of the current operation situation of the new rural cooperative medical care system in Heilongjiang Province and the successful experience of the rural medical security system at home and abroad. In order to further improve and strengthen the implementation and development of the new rural cooperative medical system in Heilongjiang Province; There are also innovations in the research process. One is to innovate from an analytical point of view and analyze the causes of the problems existing in the operation of the new rural cooperative medical system in Heilongjiang Province. This paper analyzes the defects of the new rural cooperative medical system and the obstacles hindering the operation of the new rural cooperative medical system in Heilongjiang Province. The second is to perfect Heilongjiang new rural cooperative medical system countermeasure put forward several innovation. First of all, the research status at home and abroad is systematically reviewed, the basic framework and innovation of the paper are put forward, and the related concepts and theoretical basis are clarified. Then, through a systematic analysis of the operation of the new rural cooperative medical system in Heilongjiang Province, this paper points out the problems existing in the new rural cooperative medical care system in Heilongjiang Province, such as the difficulty of raising funds, the low actual compensation, the low supervision ability and so on. And from the new rural cooperative medical system itself defects and Heilongjiang Province new rural cooperative system operation obstacles from the two angles to analyze its causes; Thirdly, by drawing lessons from the advanced experience of the foreign rural medical security system and the operation of the new rural cooperative medical care system in some areas of China, we can get some enlightenment that is conducive to the development of the new rural cooperative medical care system in Heilongjiang Province. Finally, put forward the feasible countermeasure. Combined with the actual situation in Heilongjiang Province, in the aspects of financing mechanism, supervision mechanism, compensation scheme and other related countermeasures to further improve the operation of the new rural cooperative medical system in Heilongjiang Province.
