从“碎片化”困境看农村医疗救助扶贫的政策调整

【摘要】：作为农村医疗保障扶贫的一种重要方式,农村医疗救助扶贫是有效化解"因病滞贫"难题的关键环节。如何从农村医疗救助扶贫的角度破解"因病滞贫"困局,乃是推进精准扶贫战略所面临的最大难题。然而,目前我国农村医疗救助扶贫正面临着严重的"碎片化"困境。这种"碎片化"困境主要表现为项目安排的"非整合化"、资源配置的"非均衡化"和管理体制的"非系统化",它是认知-甄别机制、供给-需求机制和权力-效率机制共同作用的结果。当前我国农村医疗救助扶贫要走出"碎片化"的实践困境,就必须推进"整体性"治理,形成结构合理、功能齐全的农村医疗救助扶贫治理体系。从"整体性"治理的政策调整目标出发,我国农村医疗救助扶贫必须着力抓好"四个度",即凸显政策的兜底度,提升项目的契合度,扩大主体的参与度,增强管理的协调度。
[Abstract]:As an important way of helping the poor by rural medical care, rural medical aid is the key link to effectively solve the problem of "stagnation of poverty due to disease". How to solve the dilemma of "stagnation of poverty due to disease" from the point of view of rural medical aid is the most difficult problem to promote the strategy of accurate poverty alleviation. However, at present, China's rural medical aid is facing a serious "fragmentation" dilemma. This kind of "fragmentation" dilemma is mainly manifested in the "non-integration" of project arrangement, the "non-equilibrium" of resource allocation and the "non-systematization" of management system, which is a cognitive-discriminating mechanism. The effect of supply-demand mechanism and power-efficiency mechanism. At present, in order to get rid of the practice dilemma of "fragmentation" of rural medical aid, it is necessary to promote "integral" management and form a system of rural medical aid and poverty alleviation with reasonable structure and complete functions. Starting from the policy adjustment goal of "holistic" governance, China's rural medical aid and poverty alleviation must pay great attention to the "four degrees", that is, to highlight the background of the policy, improve the project fit, expand the participation of the main body, and enhance the coordination of management.
