防控突发性传染病的医疗建筑网络结构研究

【摘要】:本文属国家自然科学基金面上项目“防控突发性传染病的医疗建筑网络体系构建”(51078104)的重要组成部分。所谓突发性传染病,指的是在短期内发生、有较大波及范围,出现大量的病人或死亡病例,发病率远超过常年发病率水平的一种传染病疫情;因其意外性、高危害性、广泛影响力等特点,已经成为了目前人类所面临的医学与社会的双重难题;考验着一个国家、地区、城市的公共卫生服务体系,检验着医疗建筑、医疗设施的发展均衡性。本研究从医疗建筑网络化的角度出发,对防控突发性传染病的医疗建筑网络体系下的医疗建筑规划和建筑设计进行研究,目的在于提出理论性的指导对策,并针对性的建构出防控突发性传染病的医疗建筑体系的具体操作模式。同时促进国家“坚持公益性、调动积极性”的新医改政策在疾病防控领域的具体应用,在新医改政策的框架下完善当前的医疗建筑体系,完善医疗建筑设计方法。其最终的社会意义是探讨“以人为本”的从建筑领域出发解决社会问题的新途径。本文的具体研究指向防控突发性传染病的医疗建筑网络体系,以体系中最为关键的网络结构为关注点,辅以系统论、协同论、区域科学理论等方法,创新性的提出了真实空间网络、虚拟空间网络、中介空间网络三位一体的防控突发性传染病的医疗建筑网络架构,并用网络配置计划对组成网络的医疗建筑要素进行具体的规划与设计指引,用网络运行机制对网络架构中错综复杂的内部关系进行总结。研究循着从点到面的思路,从“中观”层面构成网络的医疗建筑要素的研究介入,向上拓展到城市规划层面的医疗建筑网络体系整合,向下则具象到保证医疗建筑网络体系顺畅运行的相关技术支撑,全方位的对医疗建筑网络体系进行了梳理。在防控突发性传染病的医疗建筑网络要素的研究中,选取了整个网络体系中最为关键的四类医疗建筑“结点”:疾病预防控制中心、传染病专科医院、综合医院传染病防控空间以及基层传染病防控医疗机构,通过对近年来大量相关案例的分析,对这四类医疗建筑在网络中的职能、发展趋势以及设计要点进行梳理与阐述。在防控突发性传染病的医疗建筑网络整合模式的探索中,从政策及策略层面提出以防控突发性传染病的城市医疗建筑规划专篇为目标的垂直整合模式,主要体现为规划功能、规划技术与规划程序的整合。进而在具体方法及手段层面描述了以“联动”为特质的医疗建筑水平整合模式,并主要以哈尔滨市防控突发性传染病的城市医疗建筑规划专篇为例说明了这一特质在真实空间网络、虚拟空间网络、中介空间网络三个不同网络架构上的体现。在防控突发性传染病的医疗建筑网络技术支撑的阐释中,以突发性传染病疫情的全过程为线索,首先从广泛分析、精确控制和优化救治三个角度探索了技术支撑平台的构建方式;其次又从一体化预防、协同化控制和应急化救治三方面来明晰具体的技术支撑策略;最后还从相应的医疗技术方面,探索了其对于网络体系的辅助支持。
