EGFR基因突变与NSCLC脑转移相关性分析

【摘要】:目的:研究分析EGFR基因突变与非小细胞肺癌脑转移的相关性。方法:收集2013年12月至2016年08月于皖南医学院弋矶山医院病理科行EGFR基因检测的非小细胞肺癌患者数据共389例,选取其中220例非小细胞肺癌患者为研究对象,回顾性分析EGFR基因突变类型、常见突变与非小细胞肺癌脑转移的相关性,采用突变扩增阻滞系统法(ARMS)对所有标本进行EGFR基因检测。结果:389例非小细胞肺癌病人中,诊断为脑转移的51例(13.11%),其中19号外显子突变(23.65%)与21号外显子突变(20.82%)最常见,占所有突变数的91.05%。入选的220例非小细胞肺癌患者中,脑转移发生率为23.18%,EGFR基因突变好发于女性(p0.0001)、肺腺癌(p0.0001)病人,与病人的年龄(p=0.8933)、标本来源(p=0.2755)无关。EGFR突变型病人脑转移发生率与EGFR野生型病人脑转移发生率无明显统计学差异(p=0.6316),EGFR常见突变19del与L858R脑转移的发生率也无明显统计学差异(p=1.0000)。结论:非小细胞肺癌EGFR突变型病人脑转移发生率与EGFR野生型病人脑转移发生率无明显相关性。非小细胞肺癌EGFR常见突变19del与L858R脑转移的发生率无明显相关性。NSCLC患者BM的发生率为23.18%(51/220)。
[Abstract]:Objective: to study the correlation between EGFR gene mutation and brain metastasis in non-small cell lung cancer. Methods: a total of 389 patients with non-small cell lung cancer (NSCLC) were collected from December 2013 to August 2016, and 220 patients with NSCLC were selected. The type of EGFR gene mutation and the correlation between the common mutation and brain metastasis of NSCLC were analyzed retrospectively. The EGFR gene was detected by mutation amplification block system. Results among 389 patients with non-small cell lung cancer, 51 cases with brain metastasis were diagnosed as brain metastases, among which exon 19 mutation was 23.65% and exon 21 mutation was 20.82%, accounting for 91.05% of all mutations. In 220 patients with non-small cell lung cancer, the incidence of brain metastasis was 23.18%. EGFR gene mutation was more common in female patients with p0.0001a and lung adenocarcinoma (p0.0001). There was no significant difference in the incidence of brain metastasis between the patients with EGFR wild type and the patients with EGFR wild type. There was no significant difference in the incidence of brain metastasis between 19del and L858R. There was no significant difference in the incidence of brain metastasis between 19del and L858R. Conclusion: there is no significant correlation between the incidence of brain metastasis in patients with EGFR mutation and the incidence of brain metastasis in EGFR wild-type patients. There was no significant correlation between EGFR mutation 19del and the incidence of L858R brain metastasis. The incidence of BM in patients with NSCLC was 23.18 / 221 / 220.
