甲状腺乳头状癌组织中TSHR、NIS基因启动子区甲基化状态与临床病理特征的关系

【摘要】:目的探讨甲状腺乳头状癌(PTC)组织中促甲状腺激素受体(TSHR)与钠碘转运体(NIS)基因启动子区甲基化状态与临床病理特征的关系。方法收集80例PTC患者临床资料及手术病理标本石蜡切片,采用实时荧光定量甲基化特异性PCR(q MSP)法检测TSHR和NIS启动子区甲基化状态,分析二者与患者临床病理特征的关系。结果 PTC组织中TSHR与NIS基因启动子区甲基化发生率分别为71.25%、32.50%,癌旁正常组织中分别为8.75%、6.25%,两者比较差异有统计学意义(P均0.05);不同年龄、性别、肿瘤直径PTC患者癌组织中TSHR与NIS基因启动子区甲基化情况比较无明显统计学差异(P均0.05),病灶多发、腺外侵犯、有颈部淋巴结转移、临床分期Ⅲ~Ⅳ期PTC患者癌组织中TSHR与NIS甲基化程度高于病灶单发、局限腺内、无颈部淋巴结转移及临床分期Ⅰ~Ⅱ期者(P均0.05)。Spearman相关分析显示,PTC组织中TSHR与NIS基因启动子区甲基化呈正相关(r=0.532,P0.05)。结论 PTC组织中TSHR与NIS基因启动子区甲基化现象普遍存在,与病灶多发、颈部淋巴结转移、腺外侵犯、临床分期关系密切,两个基因启动子区域甲基化协同作用可能是引起肿瘤发展及转移的相关机制之一。
[Abstract]:Objective to investigate the relationship between the methylation status of thyroid stimulating hormone receptor (TSHR) and sodium iodide transporter (NIS) gene promoter and clinicopathological features in papillary thyroid carcinoma (PTC). Methods the clinical data and paraffin sections of 80 patients with PTC were collected. The methylation status of TSHR and NIS promoter region was detected by real-time fluorescence quantitative methylation specific PCR(q (PCR(q) method, and the relationship between them and the clinicopathological features of the patients was analyzed. Results the incidences of methylation in promoter region of TSHR and NIS gene in PTC tissues were 71.25 and 32.50, respectively, and those in adjacent normal tissues were 8.75 and 6.25.The differences were statistically significant (P < 0.05). There was no significant difference in methylation between the promoter region of TSHR and NIS gene in patients with tumor diameter PTC (P < 0.05), with multiple lesions, extraglandular invasion and cervical lymph node metastasis.
