健脾理气合剂对糖尿病胃轻瘫大鼠胃组织细胞凋亡及相关基因表达的影响

【摘要】:目的:观察"健脾理气合剂"对糖尿病胃轻瘫(diabetic gastroparesis,DGP)大鼠胃动力指标、胃组织细胞凋亡及相关基因bcl-2/bax表达的影响,探讨"健脾理气合剂"治疗DGP的效应与机理。方法:采用Wistar大鼠通过腹腔注射2%链脲佐菌素(STZ)溶液造模糖尿病大鼠,通过灌服熟地水煎液诱导DGP大鼠模型。将造模的大鼠随机分为模型组,健脾理气合剂高、低剂量组以及吗丁啉组。各组连续治疗或正常饲养3周。治疗结束后检测各项指标:包括大鼠体重、血糖、血浆胃动素浓度、胃排空率、胃平滑肌细胞凋亡以及胃组织的bcl-2/bax基因表达。结果:DGP大鼠体重较正常大鼠体重明显降低(P0.001),经健脾理气合剂及吗丁啉溶液灌胃治疗3周后,体重较模型组增加(P0.05)。DGP大鼠在各个监测点的平均血糖浓度均较正常组显著升高(P0.001),健脾理气合剂高、低剂量组平均血糖浓度均明显低于模型组(P0.05),健脾理气合剂高剂量组血糖浓度高于其他DGP大鼠(P0.05)。模型组大鼠的胃内色素残留率和血浆胃动素浓度均明显高于正常组和健脾理气合剂高剂量组(P0.05);健脾理气合剂高剂量组大鼠的胃内色素残留率和血浆胃动素浓度均低于吗丁啉组和低剂量组(P0.05),与正常组相比无显著差异(P0.05)。DGP大鼠胃壁组织中的平滑肌细胞均有不同程度的肌细胞排列紊乱及空泡样变,以模型组与吗丁啉组最明显,健脾理气合剂低剂量组次之,健脾理气合剂高剂量组最轻。DGP大鼠胃平滑肌细胞凋亡率跟正常组相比均有升高(P0.001),其中模型组明显高于健脾理气合剂高、低剂量组(P0.05),但与吗丁啉组相比无明显差异(P0.05),健脾理气合剂高剂量组的凋亡率高于其他造模各组(P0.05)。DGP大鼠胃平滑肌细胞的bcl-2/bax蛋白表达存在明显异常,而且两种基因蛋白的表达呈现负相关。模型组的bcl-2蛋白表达明显低于正常组、健脾理气合剂高、低剂量组(P0.001),但与吗丁啉组无明显差异(P0.05);健脾理气合剂高剂量组高于其他造模各组(P0.05),但低于正常组(P0.05);bax蛋白表达检测值各组比较的结果与bcl-2相同,但数值上表现为负相关,即模型组大鼠胃平滑肌细胞中bax蛋白表达明显高于正常组、健脾理气合剂高、低剂量组(P0.001),但与吗丁啉组无明显差异(P0.05),健脾理气高剂量组明显低于其他造模各组(P0.05),但低于正常组(P0.05)。结论:DGP大鼠与正常大鼠相比,体重明显减轻,血糖明显增高,存在明显的胃动力障碍。健脾理气合剂高剂量灌胃可明显增加DGP大鼠的体重、降低血糖,减少胃内色素残留率,促进血浆胃动素分泌,改善胃壁平滑肌细胞的病理变化,对糖尿病胃轻瘫具有一定的治疗作用。DGP大鼠胃壁平滑肌细胞存在bax和bcl-2基因蛋白的异常表达,两者呈现负相关。健脾理气合剂可以升高胃壁平滑肌细胞的bcl-2蛋白表达、降低bax蛋白表达,对bcl-2/bax的蛋白表达的良性调控有可能成为其治疗糖尿病胃轻瘫的作用机制之一。
[Abstract]:Objective: to observe the effect of "Jianpi Liqi mixture" on gastric dynamic index, gastric tissue apoptosis and the expression of related gene bcl-2/bax in diabetic gastroparesis rats, and to explore the effect and mechanism of "Jianpi Liqi mixture" in treating DGP. Methods: diabetic rats were induced by intraperitoneal injection of 2% streptozotocin (STZ) solution in Wistar rats, and DGP rats were induced by oral administration of cooked water decoction. The rats were randomly divided into model group, Jianpi Liqi mixture group, low dose group and morpholine group. Each group was treated continuously or fed normally for 3 weeks. At the end of the treatment, the indexes included body weight, blood glucose, plasma motilin concentration, gastric emptying rate, apoptosis of gastric smooth muscle cells and bcl-2/bax gene expression in gastric tissue. Results the body weight of DGP rats was significantly lower than that of normal rats. The rats were treated with Jianpi Liqi mixture and morpholine solution for 3 weeks. Compared with the model group, the average blood glucose concentration in the model group was significantly higher than that in the normal group, and the concentration of blood glucose in the DGP rats was significantly higher than that in the normal group, and the mixture of invigorating spleen and lipophilic qi was higher than that in the normal group. The average blood glucose concentration in the low dose group was significantly lower than that in the model group, and that in the high dose group of Jianpi Liqi mixture was higher than that in other DGP rats. The residual rate of gastric pigments and plasma motilin concentration in the model group were significantly higher than those in the normal group and the high dose group of Jianpi Liqi mixture (P 0.05), while the residual rate of the gastric pigment and the plasma motilin concentration in the high dose group of Jianpi Li Qi mixture were both higher than those in the normal group and the high dose group of Jianpi Li Qi mixture. Compared with the normal group, the smooth muscle cells in the gastric wall tissue of the DGP rats had different degrees of myocyte arrangement disorder and vacuolar degeneration, but there was no significant difference between the morpholine group and the low dose group (P 0.05), and there was no significant difference between the control group and the normal group. The model group and morpholine group were the most obvious, followed by the low dose group of Jianpi Liqi mixture. The apoptosis rate of gastric smooth muscle cells in the high dose group of Jianpi Liqi mixture was significantly higher than that in the normal group, especially in the model group, which was significantly higher than that in the model group, and the apoptosis rate of gastric smooth muscle cells in the DGP group was higher than that in the normal group. The apoptosis rate of high dose group of Jianpi Liqi mixture was significantly higher than that of other groups. The expression of bcl-2/bax protein in gastric smooth muscle cells of rats in low dose group was higher than that in other groups, but there was no significant difference compared with that in morpholine group, and the expression of bcl-2/bax protein in gastric smooth muscle cells of rats with high dose of Jianpi Liqi mixture was obviously abnormal. Moreover, the expression of the two gene proteins was negatively correlated. The expression of bcl-2 protein in the model group was significantly lower than that in the normal group. In the low dose group, there was no significant difference with the morpholine group (P 0.05), the high dose group of Jianpi Liqi mixture was higher than that of the other model making groups (P 0.05), but the result was the same as that of the normal group (P 0.05), but the value was negatively correlated with that of the normal group. The expression of bax protein in the gastric smooth muscle cells in the model group was significantly higher than that in the normal group. The expression of bax protein in the model group was significantly higher than that in the control group. The expression of bax protein in the gastric smooth muscle cells of the model group was higher than that in the normal group. Conclusion compared with the normal rats, the weight and blood glucose of the DGP rats were significantly decreased, and there were obvious gastric motility disorders. High dose of Jianpi Liqi mixture could significantly increase the weight of DGP rats, decrease blood sugar, reduce the residual rate of gastric pigment, promote the secretion of motilin in plasma, and improve the pathological changes of smooth muscle cells in gastric wall. There were abnormal expressions of bax and bcl-2 gene protein in gastric wall smooth muscle cells of diabetic gastroparesis rats. Jianpi Liqi mixture can increase the expression of bcl-2 protein and decrease the expression of bax protein in gastric wall smooth muscle cells. The benign regulation of bcl-2/bax protein expression may be one of the mechanisms of its treatment of diabetic gastroparesis.
