2016年安徽铜陵市人民医院细菌耐药性监测

【摘要】:目的了解2016年安徽省铜陵市人民医院临床所有分离细菌的分布情况及耐药性变迁。方法收集2016年所有临床分离细菌并作药敏试验,结果统一输入WHONET5.6软件统计分析。结果 2016年共收集细菌2 949株,其中革兰阴性菌2 134株(72.4%),革兰阳性菌815株(27.6%)。革兰阴性菌数量居前5位是大肠埃希菌、鲍曼不动杆菌、肺炎克雷伯菌、铜绿假单胞菌和肠杆菌属细菌;革兰阳性菌数量居前5位是凝固酶阴性葡萄球菌、金黄色葡萄球菌、粪肠球菌、屎肠球菌和链球菌属。产ESBL大肠埃希菌和肺炎克雷伯菌分别占各自菌数量的42.3%和31.1%,耐碳青霉烯类大肠埃希菌和肺炎克雷伯菌的检出率为1.2%(8/640)和29.4%(108/367),鲍曼不动杆菌对替加环素的敏感率为94.3%,对亚胺培南和美罗培南的耐药率为74.3%和74.9%,铜绿假单胞菌对哌拉西林的敏感率为78.2%,对碳青霉烯类抗生素的敏感率在70%左右;耐甲氧西林金黄色葡萄球菌分离率32.0%(74/231),耐甲氧西林凝固酶阴性葡萄球菌分离率65.6%(170/259),没有发现对替考拉宁和万古霉素耐药的葡萄球菌属;未检出替考拉宁和利奈唑胺耐药的肠球菌属细菌(粪肠球菌和屎肠球菌)。结论该院各类细菌分离数量呈逐年上升趋势,多重耐药菌的分离率逐年增长,泛耐药肠杆菌科细菌及鲍曼不动杆菌分离率的逐年增长趋势十分严峻,迫切需要继续加强医院感染管理工作。
[Abstract]:Objective to investigate the distribution and drug resistance of all clinical bacteria isolated from Tongling people's Hospital of Anhui Province in 2016. Methods all clinical bacteria isolated in 2016 were collected and tested for drug sensitivity. The results were analyzed by WHONET5.6 software. Results A total of 2 949 strains of bacteria were collected in 2016, of which 2 134 (72.4%) were Gram-negative bacteria and 815 (27.6%) Gram-positive bacteria. The first 5 gram-negative bacteria were Escherichia coli, Acinetobacter baumannii, Klebsiella pneumoniae, Pseudomonas aeruginosa and Enterobacter spp. Enterococcus faecalis, Enterococcus faecium and Streptococcus. ESBL producing Escherichia coli and Klebsiella pneumoniae accounted for 42.3% and 31.1%, respectively. The detection rates of carbapene-resistant Escherichia coli and Klebsiella pneumoniae were 1.2% (8 / 640) and 29.4% (108 / 367), respectively. The sensitivity of Acinetobacter baumannii to tegacyclin was 94.3%. The drug resistance rates of imipenem and meropenem were 74.3% and 74.9%, the sensitivity rate of Pseudomonas aeruginosa to piperacillin was 78.2%, and the sensitivity rate to carbapenem antibiotics was about 70%. The isolation rates of methicillin-resistant Staphylococcus aureus and methicillin-resistant coagulase-negative staphylococcus were 32.0% (74 / 231) and 65.6% (170 / 259), respectively. Teicoplanin and renazolamide resistant Enterococcus bacteria (Enterococcus faecalis and Enterococcus faecium) were not detected. Conclusion the number of bacteria in this hospital is increasing year by year, the isolation rate of multidrug resistant bacteria is increasing year by year, and the isolation rate of pan-drug-resistant Enterobacteriaceae and Acinetobacter baumannii is very serious. There is an urgent need to continue to strengthen the management of nosocomial infection.
