腕部尺神经管结构及尺神经卡压的解剖学研究

【摘要】: 腕部尺神经管又称Guyon管,位于小鱼际肌区的近端,豌豆骨和钩骨钩之间的一个狭窄的间隙。其中走行着尺神经、尺动脉、尺静脉等结构,然而管内空间狭小,是尺神经容易受到卡压的部分。由于腕部尺神经管存在着个体差异,就是同一个体的左右手也存在着差异,加之其结构比较复杂,位置的深浅不同,并且与周围许多结构关系密切。自1861年Guyon首先描述了腕尺管的解剖,并指出尺神经在此部位有卡压的可能以来,国内外的许多学者对该区和尺神经卡压综合症做了研究,但因该区域结构的复杂性和研究角度的不同,使腕部尺神经管的解剖学数据、尺神经卡压病因的探求方面的资料仍显不足。本文利用20具(40侧)防腐成人手臂标本,显微解剖,观察腕部尺神经管的形态结构,尺动脉的形态,对重要结构进行量化分析,并对尺神经神经走行、分支以及该区域与周围结构的关系、腕部尺神经管综合症引起的病因等问题做了研究。以期为今后的临床手术及保护重要的神经、血管提供显微解剖学依据。
[Abstract]:The ulnar canal of the wrist, also known as the Guyon canal, is located at the proximal end of the hypothenar area, a narrow gap between the pea bone and the hook. It runs along the ulnar nerve, ulnar artery, ulnar vein, etc. The ulnar nerve is liable to be compressed. Because of the individual differences in the ulnar nerve canal of the wrist, even the same individual's right and left hand are different, in addition, its structure is quite complex, and its position is different in depth and depth. Since Guyon first described the anatomy of the ulnar canal in 1861 and pointed out that the ulnar nerve might be compressed in this area, many scholars at home and abroad have studied the compression syndrome of the ulnar nerve in this area. However, due to the complexity of the structure of the region and the different research angles, the anatomical data of the ulnar nerve canal of the wrist and the exploration of the etiology of ulnar nerve entrapment are still insufficient. In this paper, 20 adult arms (40 sides) were embalmed and microdissected. The morphological structure of ulnar nerve canal and ulnar artery of wrist were observed, and the important structures were analyzed quantitatively. The walk and branch of ulnar nerve and the relationship between the region and the surrounding structure were also studied. The etiology of ulnar nerve tube syndrome in wrist was studied in order to provide microanatomical basis for clinical operation and protection of important nerves and blood vessels in the future.
