新公共服务理论视角下沈阳市老年医疗卫生服务供给与需求研究

【摘要】：医疗保险制度是中国社会保障制度的重要组成部分,随着改革开放后社会保障制度的逐步建立,医疗保险制度也日趋完善并受到社会关注。2015年国务院政府工作报告中指出:要完善城乡居民基本医疗保险,提高社会统筹层次,全面实施城乡居民大病保险制度。深化基层医疗卫生机构综合改革,加强全科医生制度建设,完善分级诊疗体系。鼓励医生到基层多点执业,发展社会办医,加快建立医疗纠纷预防调解机制。加强重大疾病防控,积极发展中医药和民族医药事业。满足人民群众的基本健康需求,不断提高医疗卫生水平,打造健康中国。人口老龄化首先出现在发达国家,1864年法国率先进入人口老龄化,之后美国、德国、日本、韩国也陆续进入到老龄化时代,二十世纪后人口老龄化问题开始在全球范围内不断加深。因此,无论在发达国家还是发展中国家,老龄化问题已然成为当今全世界社会保障领域的热点。全球都已深刻认识到老年人口占总人口的比例正悄然地以惊人的速度加大,以及由此引发的一系列的社会问题的严重性。选题的目的在于通过分析沈阳市老年医疗卫生服务供给与需求的现状及其存在问题和原因,提出相应的解决方案,高效调动并合理配置老年群体的医疗卫生服务资源,使医疗卫生服务的发展满足老年人的实际医疗需求,维护老年群体享受医疗保障的权益。运用文献综合法,搜集国内外医疗卫生服务建设方面的相关文献,为深入研究沈阳市医疗卫生服务提供翔实的基础资料;运用社会调查法,采用问卷调查和深度访谈的方式切实了解老年医疗卫生服务供给与需求的实际状况;运用比较分析和理论研究法,通过与全国重点城市的相关情况进行对比,并结合沈阳市实际状况提出有针对性的经验借鉴。采用经济模型对沈阳老年医疗卫生服务供给与需求进行预测。最后针对沈阳市老年人的实际情况,提出优化老年医疗卫生服务供给与需求的对策与建议:第一,优化政府医疗卫生服务供给,改革和完善医疗保障制度;第二,完善医疗卫生服务组织建设,重构医疗卫生管理系统;第三,合理配置医疗卫生资源,保证公民享有医疗服务的权利;第四,强化基本医疗卫生服务供给体系,促进老年护理产业发展;第五,建立医疗机构监督机制,加强卫生人才队伍建设。
[Abstract]:The medical insurance system is an important part of the social security system in China. With the gradual establishment of the social security system after the reform and opening up, In 2015, the government report of the State Council pointed out that it is necessary to improve the basic medical insurance for urban and rural residents, raise the level of social planning, and implement the serious illness insurance system for urban and rural residents in an all-round way. We will deepen comprehensive reform of primary-level medical and health institutions, strengthen the construction of the general practitioner system, and improve the hierarchical diagnosis and treatment system. Encourage doctors to practice at the grass-roots level, develop social management, and speed up the establishment of a medical dispute prevention and mediation mechanism. We will strengthen prevention and control of major diseases and actively develop the cause of traditional Chinese medicine and ethnic medicine. Meet the basic health needs of the people, constantly improve the level of medical and health care, and build a healthy China. The aging of the population first appeared in the developed countries, and in 1864, France first entered the aging population, and then the United States, Germany, Japan, and South Korea also entered the age of aging one after another. After the 20th century, the problem of population aging began to deepen on a global scale. Therefore, no matter in developed or developing countries, aging has become a hot spot in the field of social security all over the world. The world has come to realize that the proportion of the elderly population to the total population is quietly increasing at an alarming rate and the seriousness of a series of social problems arising therefrom. The purpose of this paper is to analyze the present situation of the supply and demand of medical and health services for the elderly in Shenyang and the existing problems and reasons, put forward the corresponding solutions, and efficiently mobilize and reasonably allocate the medical and health services resources of the elderly groups. The development of medical and health services can meet the actual medical needs of the elderly and safeguard the rights and interests of the elderly to enjoy medical security. Using the method of literature synthesis to collect the relevant literature of medical and health service construction at home and abroad, and to provide detailed and accurate basic data for the in-depth study of medical and health services in Shenyang; Using the method of social investigation and questionnaire survey and in-depth interview to understand the actual situation of the supply and demand of medical and health services for the elderly; By using the method of comparative analysis and theoretical research, through the comparison with the relevant situation of the key cities in China, and combining with the actual situation of Shenyang City, the author puts forward some specific experiences for reference. The economic model was used to predict the supply and demand of medical and health services for the elderly in Shenyang. Finally, in view of the actual situation of the elderly in Shenyang, this paper puts forward the countermeasures and suggestions to optimize the supply and demand of medical and health services for the elderly: first, optimize the supply of government medical and health services, reform and improve the medical security system; Second, improve the organization of medical and health services, reconstruct the medical and health management system; third, rationally allocate medical and health resources to ensure that citizens enjoy the right of medical services; Fourth, strengthen the supply system of basic medical and health services to promote the development of the elderly nursing industry; fifthly, establish the supervision mechanism of medical institutions and strengthen the construction of health personnel.
