心率变异性测量心血管机能有效性的研究

【摘要】:目的:研究心率变异系数与哈佛台阶试验和30秒钟30次蹲起实验测量的心血管机能水平之间是否存在相关性;探索以哈佛台阶试验测量为效标,心率变异性测量心血管机能的有效性是否比30秒钟30次蹲起实验测量心血管机能的有效性高;旨为实时预报有可能发生的运动风险和运动猝死提供理论和实践参考依据。方法:选取符合实验要求的受试者32名,按照WHO最新年龄划分标准,编为青年组和中年组;选用国际上公认的、简易可行的用于测量心血管机能指数的哈佛台阶试验和30秒钟30次蹲起实验,以有效性较高的30秒钟30次蹲起实验的测量为对照,测得受试者的心率、血压、台阶指数、心功指数和心电信号等指标;以受试者30秒钟30次蹲起实验以及3分钟哈佛台阶试验中同步记录的、具有心率变异特征的心电信号为重点研究部分,实验结束后对所得数据进行统计分析。结果:(1)相关性的实验结果:青年组心功指数与台阶指数的相关系数为-0.656(P=0.006),心率变异系数(120-180秒)与台阶指数的相关系数为-0.837(P=0.000);中年组心功指数与台阶指数的相关系数为-0.587(P=0.017),心率变异系数(120-180秒)与台阶指数的相关系数为-0.754(P=0.001)。(2)均以哈佛台阶试验为效标,测量有效性的实验结果:青年组心功指数与台阶指数的等级相关系数为-0.603(P=0.013),心率变异系数(120-180秒)与台阶指数的等级相关系数为-0.968(P=0.000);中年组心功指数与台阶指数的等级相关系数为-0.590(P=0.016),心率变异系数(120-180秒)与台阶指数的等级相关系数为-0.897(P=0.008)。结果显示,青年组和中年组的心率变异系数与台阶指数的等级相关性均高于心功指数与台阶指数的等级相关性。结论:(1)无论是青年组还是中年组,心率变异系数分别与心功指数和台阶指数所表示的心血管机能水平之间存在相关性,且心率变异系数与台阶指数的相关性高于心功指数与台阶指数的相关性,说明使用心率变异性指标测量心血管机能得到了实验结果的支持,是有理论和实践意义的。(2)无论是青年组还是中年组,以哈佛台阶试验的测量为效标,使用心率变异性指标测量心血管机能的有效性比使用30秒钟30次蹲起实验测量心血管机能的有效性高,说明运用运动中实时的心率变异性测量心血管机能的有效性能够达到实际的要求。
[Abstract]:Objective: to study the correlation between heart rate variability (HRV) and cardiovascular function in the Harvard step test and 30 squat up test in 30 seconds. The effectiveness of heart rate variability in measuring cardiovascular function is higher than that in 30 seconds squat up experiments, which provides a theoretical and practical reference for the real-time prediction of possible exercise risk and sudden death during exercise. Methods: 32 subjects who met the requirements of the experiment were divided into youth group and middle age group according to the latest age classification standard of WHO. The simple and feasible Harvard step test for measuring cardiovascular function index and the 30 squat up test for 30 seconds were used to measure heart rate, blood pressure and step index. The study focused on ECG signals with heart rate variability, which were recorded synchronously in 30 squats in 30 seconds and 3 minutes in the Harvard step test. At the end of the experiment, the data were statistically analyzed. Results: the correlation coefficient between heart function index and step index in young group was -0.656, and the coefficient of heart rate variability was 120-180 seconds.) the correlation coefficient between heart function index and step index was -0.837, and the correlation coefficient between heart function index and step index was -0.837, the correlation coefficient between heart function index and step index was -0.837. The correlation coefficient between the HRV and the step index was -0.754 ~ P0. 001g ~ (2).) the Harvard step test was used as the effective standard. The results showed that the correlation between the coefficient of heart rate variability and the step index was higher in the young group and the middle age group than in the heart function index and the step index. Conclusion (1) there is a correlation between the coefficient of heart rate variability and the cardiovascular function level expressed by the cardiac work index and step index, both in the young group and in the middle age group. The correlation between heart rate variability and step index was higher than that between heart function index and step index, which indicated that the use of heart rate variability index to measure cardiovascular function was supported by the experimental results. Using heart rate variability to measure cardiovascular function was more effective than using 30 squats in 30 seconds to measure cardiovascular function. It shows that real-time heart rate variability in exercise can meet the practical requirements.
