【摘要】:目的:探讨Ensite Navx三维标测系统指导下射频消融治疗左侧旁道的有效性及安全性。方法:选取2014年6月至2016年12月期间经房间隔穿刺行射频消融治疗的左侧旁道患者91例,将其分为观察组和对照组,观察组45例患者在Ensite Navx三维标测系统引导下,必要时结合短暂X透视进行射频消融治疗;对照组46例患者在传统X线透视下行射频消融治疗。观察指标:(1)放置标测导管过程中X线照射时间和曝光剂量;(2)操作消融导管进行标测消融过程中X线照射时间和曝光剂量;(3)手术时间;(4)即刻成功率;(5)并发症。结果:(1)放置标测导管过程中两组间X线照射时间及照射剂量:观察组X线照射时间(2.4±1.4)min VS对照组(2.5±1.2)min,两组间无显著差异,(P0.05);观察组X线曝光剂量(1.6±1.1)mGy与对照组(1.5±1.0)mGy相比,两组间也无显著差异,(P0.05)。(2)消融导管操作过程中两组间X线照射时间及照射剂量:在操作消融导管过程中,观察组的X线曝光时间(5.1±2.3)min VS对照组(13.1±3.1)min,观察组的X线曝光时间明显低于对照组,观察组X线曝光时间较对照组缩短61%(P0.05);同时观察组X线曝光剂量(5.7±2.6)mGy较对照组X线曝光剂量(17.8±4.3)mGy也显著减少,观察组X线曝光剂量较对照组下降68%,(P0.05)。(3)手术时间:观察组(59.1±18.8)min VS对照组(62.3±20.6)min,两组间屋显著差异(P0.05)。(4)即刻成功率:观察组全部消融成功,对照组1例左后游离壁旁道未消融成功,后借助Ensite Navx三维系统消融成功,术后随访3个月观察组无复发病例。(5)并发症:其中观察组出现单纯穿刺口血肿1例,对照组出现气胸1例,两组诉心前区或后背部不适患者4例,分别予相应处理。结论:Ensite Navx三维系统引导下行左侧旁道射频消融,可明显缩短操作消融导管过程中X线照射时间,减少X线曝光剂量,进而减少手术全程的X线透视时间和曝光剂量;手术成功率与传统X线透视下射频消融无差异;并不增加手术相关并发症的发生风险。
[Abstract]:Objective: To investigate the Ensite Navx RF carto guided ablation is effective and safety in the treatment of left accessory pathways. Methods: during the period from June 2014 to December 2016 with atrial septal puncture underwent radiofrequency ablation treatment in patients with 91 left accessory pathways were divided into observation group and control group, 45 patients in observation group were Ensite Navx three mapping system under the guidance, when necessary and short-term perspective X radiofrequency ablation therapy; 46 patients in the control group had conventional X-ray fluoroscopy radiofrequency ablation treatment. Observation index: (1) X-ray irradiation time and exposure dose mapping catheter placed in the process; (2) the operation of the ablation catheter of X-ray radiation exposure and mapping the ablation process dose; (3) the operation time; (4) immediate success rate; (5) complications.
