中国精神分裂症患者服药依从性影响因素的Meta分析

【摘要】：目的:评价中国精神分裂症患者服药依从性影响因素,为临床提高患者服药依从性提供支持和指导意见。方法:从中英文数据库中检索有关中国精神分裂症患者服药依从性影响因素的文献进行分析。通过英文检索词 "Schizophrenia*","Schizophrenic Disorder*","MedicationAdherence","Medication Persistence","Medication Compliance","Risk Factor*","Population*at Risk","China","Mainland China","Hong kong","Macao","Taiwan"和中文检索词"精神分裂症"、"精神分裂性障碍""药物治疗依从性"、"服药依从性""危险因素"、"影响因素"对万方数据库、中国知网(CNKI)、中国生物医学文献数据库(CBM)、Pubmed、Web of Science、Cochrane 和 Scopus 这 7 个国内外数据库进行文献检索。同时,对新期刊以及新文献进行手工检索。经质量评估后,收集数据。两名研究人员分别独立进行,不一致的信息由第三位研究人员再次核对。采用Review manger 5.3软件对服药依从性的影响因素进行合并分析。采用优势比(odds ratio,OR)和标准化均数差值(standardized mean difference,SMD)对分类指标和连续性指标分别衡量。并且,对异质性≥50%的因素采用合并值敏感性分析法,发表偏倚采用漏斗图进行检验。结果:本研究最终纳入9篇文献,合计分析了 12个指标,包括:患者年龄(岁)、性别、婚姻状况、病程(年)、社会支持、药物副作用、受教育程度(≥9年)、工作情况、支付能力、服用的药物种数、医患关系以及患者的自知力。本研究显示4个因素与服药依从性关联有统计学意义,分别是社会支持:OR=3.19,95%CI:1.18～8.64,PQ0.00001,P=0.02;药物副作用:OR=0.35,95%CI:0.18～0.68,PQ=0.0001,P P=0.002;支付能力:OR=1.92,95%CI:1.09～3.40,PQ=0.0006,P=0.02;患者自知力:OR=5.51,95%CI:1.42～21.42,PQ=0.05,P=0.01。结论:中国精神分裂症患者服药依从性的主要影响因素为:社会支持、药物副作用、支付能力及患者自知力。
