温阳通便法治疗肾阳虚便秘机理研究

【摘要】:目的运用活性炭联合冰水和5%乙酸法,制造大鼠肾阳虚便秘动物模型,探究温阳通便法治疗肾阳虚便秘与胃动素(MTL)、P物质(SP)和生长抑素(SS)水平以及与cAMP、cGMP、T3、T4的关系。方法60只SD大鼠随机分为5组,即空白组,模型组,济川煎高、中、低剂量组,每组12只,把济川煎高(3.8g/kg)、中(1.9g/kg)、低(0.95g/kg)剂量组,分别配制成25.2、12.6、6.3%的浓度,5%乙酸合并活性炭冰水造模4周后,各组按1.5ml/100g灌胃给药,模型组给予等体积生理盐水,每天1次,连续7天。观察大鼠的一般生理变化,记录大鼠首粒排便时间及其6小时内排便数量,以及大鼠结肠的肌电和HE染色病理切片,分别用Elisa法和免疫组化法(SABC法)检测大鼠血清和结肠中MTL、SP和SS水平等实验,并用Elisa法测定大鼠血清cAMP、cGMP、T3、T4含量。结果结肠病理切片各实验组均未见差异。造模后大鼠一般生理学特征为:腹胀、披毛变硬、身体蜷缩、体重变轻、大便色黑干燥。与空白组对比,模型组的大鼠排便时间延长,排便粒数减少,胃肠蠕动减弱,血清和结肠中MTL水平降低,P物质不变,SS含量升高。治疗组的上述体征和指标有明显改善,其中济川煎高、中剂量组与济川煎低剂量组比较,疗效更明显。此外还观察到了济川煎能够明显纠正肾阳虚便秘模型动物cAMP、cGMP失衡的作用,但本模型T3、T4无明显变化。结论活性炭联合冰水和5%乙酸法造模后,结肠没有发生器质性病变,属于功能性便秘,大鼠各体征表现符合肾阳虚证候,所造出的大鼠便秘模型是肾阳虚便秘模型。济川煎治疗肾阳虚便秘机理与提高MTL水平,降低SS水平有密切关系,与P物质关系不大,且济川煎高、中剂量组治疗肾阳虚便秘效果更加显著,济川煎还通过纠正本模型动物cAMP、cGMP的失衡而发挥改善肾阳虚的功能,体现了标本兼治的作用特点。
[Abstract]:Objective to establish an animal model of kidney-yang deficiency constipation in rats by using activated carbon combined with ice water and 5% acetic acid, and to explore the relationship between the treatment of kidney-yang deficiency constipation and motilin motilin (motilin) and somatostatin (SS), and the relationship between them and cAMPG MPT _ (3) T _ (3) T _ (4). Methods Sixty Sprague-Dawley rats were randomly divided into 5 groups: blank group, model group, Jichuan decoction high, middle and low dose groups, 12 rats in each group. The rats were divided into 3. 8 g / kg, 1. 9 g / kg, 0. 95 g / kg and 0. 95 g / kg, respectively. After 4 weeks of modeling with 5% acetic acid and activated carbon ice water, each group was given intragastric administration of 1.5ml/100g, and the model group was given normal saline of equal volume once a day for 7 days. To observe the general physiological changes of rats, to record the first defecation time and the number of defecation within 6 hours, and to observe the myoelectric and HE staining pathological sections of rat colon. The levels of MTLS-SP and SS in serum and colon of rats were detected by Elisa method and immunohistochemical method, respectively. The content of cAMP-cGMP-T3T3T4 in serum of rats was measured by Elisa method. Results there was no difference in colonic pathological section among the experimental groups. The general physiological characteristics of the model rats were abdominal distension, drape, body curling, light weight, dry stool color and black. Compared with the blank group, the defecation time of the model group was prolonged, the defecation granule number was decreased, the gastrointestinal peristalsis was weakened, the MTL level in serum and colon was decreased and the content of SS was increased. The above signs and indexes in the treatment group were obviously improved, among them, the effect of Jichuan decoction was higher than that of the middle dose group and the Jichuan decoction low dose group. In addition, it was observed that Jichuan decoction could obviously correct the imbalance of cAMPN cGMP in animal model of kidney yang deficiency constipation, but there was no obvious change of T3T4 in this model. Conclusion the model of activated carbon combined with ice water and 5% acetic acid has no organic lesion and belongs to functional constipation. The symptoms of each sign of rats accord with the syndrome of deficiency of kidney yang. The model of constipation in rats is the model of constipation due to deficiency of kidney yang. The mechanism of Jichuan decoction in treating constipation of kidney-yang deficiency was closely related to raising MTL level and decreasing SS level, but had little to do with substance P, and Jichuan decoction had a higher effect on constipation of kidney-yang deficiency, and the effect of medium dose group on constipation of kidney-yang deficiency was more remarkable. Jichuan decoction also improved the function of kidney yang deficiency by correcting the imbalance of cAMP and cGMP in the model animal, and reflected the function of both symptoms and symptoms.
