基于数据挖掘的葛根芩连汤方证研究

【摘要】:研究目的本论文主要包含两大部分内容:其一、葛根黄芩黄连汤方证的文献综述、理论研究、现代临床应用与实验室研究。其二、葛根黄芩黄连汤的循证医学与系统研究。全面检索、收集、阅读、统计、筛选出葛根黄芩黄连汤的随机对照试验临床研究文献,从结果中发现葛根黄芩黄连汤在临床上主要用来治疗小儿轮状病毒性肠炎。在此结果基础上,具有相同研究内容与目地之研究文献进行分类,依据文献资料共分为2组。使用Cochrane系统提供的Meta-analysis统计软体对2组数据进行统计分析。最后,遵循循证医学对每个疾病提出最佳的治疗方案原则,小儿轮状病毒性肠炎设计出一套建议治疗方案。研究对象1.发表于1949年-2017年03月之间的葛根黄芩黄连汤的期刊文献。2.葛根黄芩黄连汤、中西医结合治疗小儿轮状病毒性肠炎的随机对照试验期刊文献。研究方法1.题目确立:基于中医学辨证论治之基础与西医内科学对小儿轮状病毒的定义,研究题目确立为"葛根黄芩黄连汤治疗小儿轮状病毒性肠炎随机对照试验之RevMan Meta分析评价"。2.文献资料检索方法:6个主题词:葛根黄芩黄连汤、葛根芩连汤、葛根黄芩黄连加减方、复方葛根黄芩黄连汤、葛根芩连汤加减方、复方葛根芩连汤。2个关键词:小儿轮状病毒、小儿轮状病毒治疗。3.数据处理:使用Cochrane系统提供的Meta-analysis统计软体对2组数据进行统计分析。研究内容使用Cochrane系统提供的Meta-analysis统计软体分析葛根黄芩黄连汤治疗小儿轮状病毒性肠炎之二分类变量(dichotomous)、连续性变量(continuous)数据分析。1.二分类变量:1.1葛根黄芩黄连汤vs单纯西医基础疗法1.2葛根黄芩黄连汤+西医基础疗法VS西医基础疗法2.连续性变量:中西医结合治疗VS西医基础疗法之小儿轮状病毒性肠炎的临床症状治愈时间比较分析:2.1止泻时间SMD2.2退热时间SMD2.3呕吐消失时间SMD2.4脱水纠正时间SMD2.5腹胀消失时间SMD2.6总治疗疗程时间SMD研究结果使用Cochrane系统提供的Meta-analysis统计软体分析葛根黄芩黄连汤治疗小儿轮状病毒性肠炎之治疗疗效,二分类变量(dichotomous)、连续性变量(continuous)数据分析统计结果如下:1.二分类变量:1.1葛根黄芩黄连汤单纯西医基础疗法1.2葛根黄芩黄连汤+西医基础疗法西医基础疗法2.连续性变量:2.1中西医结合治疗之止泻时间SMD西医基础疗法之止泻时间SMD2.2中西医结合治疗之退热时间SMD西医基础疗法之退热时间SMD2.3中西医结合治疗之呕吐消失时间SMD西医基础疗法之呕吐消失时间SMD2.4中西医结合治疗之脱水纠正时间SMD西医基础疗法之脱水纠正时间SMD2.5中西医结合治疗之腹胀消失时间SMD西医基础疗法之腹胀消失时间SMD2.6中西医结合治疗之总治疗疗程时间SMD西医基础疗法之总治疗疗程时间SMD研究成果与小儿轮状病毒性肠炎建议治疗方案结论就此统计结果,笔者与临床经验丰富且治疗疗效卓越导师陈明教授对小儿轮状病毒设计出一套建议治疗方案,方案如下:1.中药处方:葛根黄芩黄连汤(葛根、黄芩、黄连、甘草)2.兼证配伍:2.1脾虚:白术、山药、白扁豆2.2湿胜:苍术、泽泻、茯苓、车前子2.3脾胃气滞:木香、柴胡2.4脾胃虚寒:白术、干姜2.5呕恶苔腻:半夏、藿香2.6热毒血痢:金银花炭3.西医基础疗法:纠正电解质及酸碱平衡紊乱、营养支持。
[Abstract]:This paper mainly contains two main parts: first, the literature review, the theoretical study, the modern clinical application and the laboratory study of Gegen Huanglian decoction, the second, the evidence-based medicine and the systematic study of Gegen Huanglian decoction. The random control trial of the Radix Scutellariae Decoction of Radix Scutellariae is selected. The results of the results showed that kudzu Scutellaria Huanglian soup was mainly used to treat children with rotavirus enteritis. On the basis of the results, 2 groups were divided into 2 groups according to the literature. The Meta-analysis software provided by the Cochrane system was used for the 2 groups of data. Finally, according to the principle of the best treatment plan for each disease by evidence-based medicine, a set of recommended treatment scheme was designed for children with rotavirus enteritis.
