热刺激微创取出骨结合牙种植体实验研究

【摘要】:目的利用动物实验研究热刺激微创取出骨结合牙种植体的条件,建立微创取出骨结合牙种植体动物模型,为临床微创取出废用牙种植体研究奠定基础。方法研究分为三个部分:一、建立骨结合的牙种植体动物模型:选用5只1-2岁杂种犬为实验动物,拔除两侧下颌第三或第四前磨牙,两个月后常规种入牙种植体,三个月后经临床及放射学检查,确定骨结合形成;二、研究热刺激取出骨整合牙种植体条件:5只骨结合牙种植体动物模型,随机选取右侧为实验侧,组成实验组,分别以48±1℃持续1min(简称A因素)、53±1℃持续2.5min(B)、59±1℃持续3.5min(C)刺激实验侧种植体,于每次热刺激前及刺激后4周测量种植体稳定性系数(implant stability quotient,ISQ),改良菌斑指数(modified plaque index,mPLI),探诊的深度(probing depth,PD),观察影像学表现以及能否用扭矩扳手(≥35Ncm)取出种植体,左侧为对照侧,组成对照组,不施加任何热刺激,测量项目同实验组。实验结束后随机选取一只实验动物取双侧下颌骨标本,观察比较双侧骨结合变化情况;三、热刺激法与传统取骨环钻法取出骨结合牙种植体牙槽骨骨量变化比较:右侧以热刺激法去除种植体,左侧同期以取骨环钻法去除种植体,然后测量植体取出术前、术后两个月、术后三个月双侧牙槽骨骨量变化(宽度、高度),采取SPSS22.0统计软件对实验数据进行统计分析,比较双侧牙槽骨骨量的变化。结果成功建立了热刺激微创取出骨结合牙种植体动物模型:A、B因素刺激前和刺激后4周,实验侧与对照侧相比,ISQ、mPLI,PD无显著性差异,放射影像学检查无明显变化,无法用扭矩扳手取出种植体。C因素刺激后4周同对照侧相比,实验组ISQ明显降低,差异具有显著性(P0.05),放射影像学检查显示种植体周围出现线形低密度影,但mPLI,PD双侧没有显著性差异(P0.05),实验组采用50Ncm的力量可以取出种植体,对照组无法取出,病理结果示该条件下实验侧种植体表面出现有限骨质吸收破坏。在种植体取出术之后两个月,下颌双侧牙槽骨基本愈合,同取出术前相比,实验组牙槽嵴宽度和高度未见显著性变化(P0.05)。取出术后三个月实验侧牙槽嵴宽度和高度无显著性变化(P0.05),对照侧牙槽嵴宽度和高度下降明显(P0.05)。取出术后三个月及术后两个月,牙槽嵴高度组内比较差别没有显著性(P0.05)。结论动物实验显示,已完成骨结合的牙种植体在59±1℃持续3.5min热刺激条件下可使其周围骨质出现有限破坏,从而破坏其骨结合,而周围软组织无明显损伤,达到微创取出种植体的目的,成功建立了热刺激取出骨结合牙种植体动物模型,为进一步研究临床废用牙种植体的微创取出提供参考。
