网状Meta分析报告量表的制定与验证

【摘要】:背景和目的:网状Meta分析可以比较同一疾病多个干预措施的有效性和安全性,基于排序结果筛选最佳干预措施。研究显示网状Meta分析的报告质量较低,影响其生产证据的使用。目前网状Meta分析报告量表并不全面,不同报告量表间的差异较大,不能有效规范网状Meta分析的报告。因此有必要制定全面规范详细的网状Meta分析报告量表,并采用科学的方法进行验证。方法:基于系统全面的文献检索,纳入英文网状Meta分析,采取社会网络分析和文献计量学方法分析其应用现状;比较网状Meta分析的报告量表,分析不同量表间的差异,评估其全面性;纳入涉及网状Meta分析报告的文献或量表,整合网状Meta分析的报告条目;随机抽取25%的网状Meta分析,以整合后的报告条目评估其报告质量,进行探索性因子分析制定网状Meta分析的报告量表;纳入外科学网状Meta分析,以该量表评价其报告质量,并进行信度分析。采用SPSS 21.0软件进行统计分析,信度分析主要测量Cronbach α系数、Spearman-Brown系数和Guttman分半系数。Cronbach α系数0.8,表示量表的内部一致性很高,信度较好。Spearman-Brown系数和Guttman分半系数大于0.7就认为量表信度较高。结果:纳入522篇网状Meta分析。网状Meta分析的发表量逐年上升,大多数关注慢性疾病的药物治疗。共纳入6个网状Meta分析的报告量表,比较发现,6个量表均不全面,对网状Meta分析的报告要求并不一致。大部分量表只要求报告方法和结果部分,并不要求报告背景和讨论。基于15篇文献的系统评价,共收集47个网状Meta分析报告条目,并与PRISMA报告条目进行整合,最终获取55个报告条目。采用此量表评估25%的网状meta分析(113篇),结果显示网状Meta分析的报告并不充分和全面,尤其是方法学部分:45篇(39.82%)网状Meta分析清楚地报告研究筛选过程,68篇(60.18%)网状Meta分析清楚地报告资料提取过程,61篇(53.98%)网状Meta分析清楚地报告质量评价过程。10篇(8.85%)网状Meta分析清楚地报告临床和方法学异质性的评估,54篇(47.8%)网状Meta分析清楚地报告统计学异质性的评估和处理方法,7篇(6.19%)网状Meta分析清楚地报告相似性评估方法,18篇(15.93%)网状Meta分析清楚地报告不一致性的检测和处理方法。基于报告质量评估结果的探索性因子分析,获取34个网状Meta分析报告条目,即网状Meta分析的报告量表。以73篇外科学网状Meta分析(涉及6种治疗措施和11种疾病)的报告质量为基础进行信度分析,Cronbach α值为0.84,即该量表可以获取84%网状Meta分析真实的报告情况;Spearman-Brown系数为0.82, Guttman分半系数为0.82。结论:网状Meta分析的报告质量较差,特别是关键部分报告不充分。基于系统评价和探索性因子分析制定的网状Meta分析报告量表含34个条目,信度分析显示其具有较高信度,可以用来规范网状Meta分析。但未来研究仍需要在其他领域进一步验证网状Meta分析报告量表的信度和效度。
[Abstract]:BACKGROUND & OBJECTIVE : A meta - analysis of mesh Meta - analysis can compare the effectiveness and safety of multiple intervention measures of the same disease , and screen best interventions based on ranking results .
To compare the report scale of mesh meta - analysis , to analyze the difference between different scales and to evaluate its comprehensiveness ;
include a literature or scale related to a mesh meta - analysis report , an integrated mesh meta - analysis report entry ;
A mesh meta - analysis of 25 % was randomly selected to assess the quality of its report with the integrated report entry , and a report scale of mesh meta - analysis was developed for exploratory factor analysis ;
A meta - analysis was conducted with SPSS 21 . 0 software . The results showed that the reliability of the scale was high and the reliability was good . The results showed that there were only a total of 47 mesh Meta - analysis reports .
Conclusion : There are 34 entries in the mesh meta - analysis report scale based on systematic evaluation and exploratory factor analysis . The reliability analysis shows that it has higher reliability and can be used to standardize the mesh meta - analysis . However , future research still needs to further verify the reliability and validity of the mesh meta - analysis report scale in other fields .
