热刺激取出骨结合种植体动物模型的建立

【摘要】:目的:研究热刺激取出骨结合牙种植体的条件,建立热刺激微创取出骨结合牙种植体的动物模型。方法:选用4只1~2岁杂种犬为实验动物,拔除双侧下颌第三或第四前磨牙,2个月后常规植入牙种植体,3个月后经临床及放射学检查,确定骨结合形成,随机选取右侧为实验侧,组成实验组,分别以(48±1)℃持续1min(简称A因素)、(53±1)℃持续2.5min(B)、(59±1)℃持续3.5min(C)刺激实验侧种植体,于每次热刺激前及刺激后4周测量种植体稳定性系数(ISQ),改良菌斑指数(modified plaque index,mPLI),探诊深度(probing depth,PD),观察影像学表现以及能否用扭矩扳手取出种植体,左侧为对照侧,组成对照组,不施加任何热刺激,测量项目同实验组。采用SPSS22.0统计软件对实验数据进行统计分析。结果:A、B因素刺激前和刺激后4周,实验组与对照组相比,ISQ、mPLI,PD无显著性差异,影像学无明显变化,无法用扭矩扳手取出种植体。C因素刺激后4周与对照组相比,实验组ISQ明显下降,差异具有显著性(P0.05),根尖X片显示种植体周围出现线形低密度影,低密度影外侧骨密度略有增高,但mPLI,PD实验组和对照组无显著性差异,实验组采用50Ncm的扭矩可以取出种植体,对照组无法取出。种植体取出术后2个月,实验组牙槽骨基本愈合,与取出术前相比,牙槽嵴宽度和高度无显著性变化。结论:动物实验显示,已完成骨结合的种植体在(59±1)℃持续3.5 min热刺激条件下可使其周围骨质出现有限破坏,从而破坏其骨结合,达到微创取出种植体的目的,成功建立了热刺激取出骨结合牙种植体动物模型。
[Abstract]:Objective: to study the condition of extraction of osseous and dental implants by thermal stimulation, and to establish an animal model of osseous and dental implants taken out by thermal stimulation. Methods: four mongrel dogs aged 1 or 2 years were selected as experimental animals. Bilateral mandibular third or fourth premolars were extracted and implanted into dental implants 2 months later. After 3 months clinical and radiologic examinations were performed to determine the formation of bone union.
