【摘要】:目的:通过测定血清胰岛素样生长因子I(IGF-I)及胰岛素样生长因子结合蛋白3(IGFBP-3),探讨IGF-I、IGFBP-3与胎儿宫内生长发育和生后生长追赶的关系及其影响因素。方法:选自2004年12月至2006年2月在我院住院的新生儿病例,共64例,分为两组:①观察组:为宫内发育迟缓(intrauterine growthretardation,IUGR)儿,共39例,胎龄在32+1周至41周,其中足月儿28例,早产儿11例。②对照组:为适于胎龄(appropriatefor gestational age,AGA)儿,共25例,胎龄在34周至40+3周,其中足月儿16例,早产儿9例。收集两组患儿相关病史,出生时情况,并对观察组于生后6~12个月进行随访,分别于生后7天及随访时采集静脉血3毫升,以3000RPM离心取血清于-20℃冰箱冰冻待检,应用酶联免疫吸附方法(ELISA)测定标本IGF-I和IGFBP-3的浓度。结果:观察组新生儿静脉血IGF-I水平(25.15±1.19ng/dl)及IGFBP-3水平(1.91±0.67ng/dl)明显低于对照组(27.20±8.66)和(6.40±1.25),同一指标两组间比较差异有显著性(P0.05);其中早产儿IGF-I水平(28.65±9.81ng/dl)和IGFBP-3水平(1.45±0.26ng/dl)明显低于足月儿(41.77±10.58ng/dl)和(4.10±1.29ng/dl),同一指标两者比较差异显著(P0.05);IGFBP-3与IGF-I呈显著性正相关(P0.001);IGF-I、IGFBP-3与出生体重、身长和头围呈正相关(P0.01);母亲有妊娠高血压疾病史与无有妊娠高血压疾病史者比较,其新生儿血清IGF-I和IGFBP-3水平均降低(P0.05);胎儿有脐带绕颈史及宫内窘迫或窒息史者血清
[Abstract]:Objective: to study IGF-I by measuring serum insulin-like growth factor (IGF-I) and insulin-like growth factor-binding protein (IGFBP-3). The relationship between IGFBP-3 and fetal intrauterine growth and postnatal growth catch-up and its influencing factors methods: from December 2004 to February 2006, the neonatal cases in our hospital were selected. 64 cases were divided into two groups: intrauterine intrauterine growth rate (IUGR) group (39 cases). The gestational age ranged from 321 weeks to 41 weeks, of which 28 were term infants. There were 11 premature infants (n = 11) and 25 cases (n = 25) in the control group, which were suitable for gestational age and approved for gestational age AGA (n = 25). The gestational age ranged from 34 weeks to 403 weeks, including 16 full-term infants and 9 premature infants. The related history and birth time of the two groups were collected, and the observation group was followed up from 6 to 12 months after birth.
